Your picture suggest strong mandibular ligament (marionette lines) which may be a factor in early recurrence of the jowls. You can try liposuction and even pre jowl sulcus filling temporarily but I think the best result would be facelift revision at one year post-op. I would approach it in a "deep plane" fashion.

The real answer is "depends on who you ask...". I prefer facelifting in younger patients 40s for three reasons: 1) You enjoy your results earlier...instead of waiting to get it done later. 2) When you are younger, your result generally lasts longer because tissue will always "sag" at some point (fact of life). It depends on the suture material. If your doctor said it will dissolve then most likely it's an absorbable type. They can remain for up to 6 months. You can try massaging and steroids but can risk surface irregularity. If they're only palpable and not visible I would let TIME take its course.

With good wound care, the area will improve over time. Judging from the size, it may take several weeks. The face does have incredible healing capacity to the point where scarring maybe minimal and the affected area may be lifted again and trimmed in about one year post op.
Are there other solutions to tightening up this skin without a facelift?

The superficial neck soft tissue has several "layers". The platysma is the muscle layer underneath the skin and soft tissue. Tightening the muscle (platysmaplasty) only addresses the muscle but the skin remains loose and crepy unless the skin is also "treated" to some extent.

I can definitely see your concerns. Without examining you and judging only from static view, you maybe a candidate for revision liposuction, and chin augmentation to balance your profile. You may also have descended submandibular gland which makes a crisp jawline more difficult to achieve without removing parts of the gland. The mild jowling on the side profile view is best treated with a revision facelift. The hollowness of the lower lids can be addressed...

Without examining you and only looking at static pictures, I have seen worse nasal tip depression when smiling. Cutting the depressor may not achieve much given your degree of depression which I think is minimal. Be careful on the lip lift as it may pull down the nasal base as it's lifting up the upper lip. Judging from your pictures, you may benefit from conservative upper eyelid skin removal (blepharoplasty), deep plane facelift (targeting laugh lines and marionette lines), and neck/submental liposculpting/liposuction, and good skin care (start with retin-A and slowly titrate up). If you are not ready for surgery, fillers will produce good results in the laugh and marionette lines and even pre jowl region to camouflage the jowling.

Hard to be sure exactly the cause without examining the region. If the surgeon said it was caused by too much skin being removed then the most likely reason is scar contraction. Depending on skin laxity in that region a revision face or neck lift may be able to "recruit" more normal skin to re-drape and re-create the sulcus for you.

Swelling under eye after facelift surgery or any facial surgery in the middle and lateral cheek/nose/forehead can happen due to altered lymphatic flow/drainage as a result of the facelift. As the face de-swells the drainage will improve.

The darkened areas requires immediate attention. I am worried that region of skin may be ischemic and necrotic. The good news is that it's around the ear which can be addressed later with another "tuck up" if the skin does not heal well and scar contracture ensues.

Facial fat grafting using your own body fat (abdomen and thigh) is a viable option. I use a micro fat injection gun which minimizes bumps. This procedure can be done in the office with minimal discomfort and quick recovery in that region of the face. You are also a good candidate because you appear young and the skin is taunt and "younger" fat that is transferred tend to survive more consistently.

It's hard to say with just one view but I recommend mini-lift or short scar facelift because you do not have excess neck fat or hanging skin. Your eyes deserve more attention as they are deep set but you may like that look. If you would like more fullness and volume around the eyes, taking your own fat and grafting them conservatively for a more sensual look is another option.

This is almost the norm with aging face surgery simply because you are addressing the eyes which can have a dramatic improvement in your look AND the jowls and tightening up the neck soft tissue for overall balanced youthful appearance. You also have one recovery as opposed to separating the procedures.

If you know at some point in your lifetime you will want a facelift, it's better to do it while you are young because the results truly do last longer AND you enjoy your results sooner. You have mild laugh lines and marionette lines which can be treated with fillers, however, you are only "renting" the result with fillers and more permanent results can be achieved with a deep plane facelift.
