It is true that thicker skin can limit tip definition in rhinoplasty. The photos show a thick skinned nasal tip with cephalic orientation of the lower lateral cartilages, accentuating the width of your nose. In such circumstances, the lower lateral (tip) cartilages can be conservatively reduced in size (7-8 mm is a safe width), and additional suture techniques are utilized to address the cephalic orientation. Reducing the size of the cartilages alone may not be enough.

This is an issue that you will need to discuss with your Internist, because you cannot have a facelift while on warfarin or heparin. With portal vein thrombosis, you may be considered too high a risk for elective surgery. Some of the anti-platelet agents(dabigatran/ pradaxa) have a shorter half life and can be stopped up to 48 hours prior to surgery. However, whether or not you are suitable to make such a change is up to your medical team.

In my hands, an uncomplicated primary facelift takes about 3 hours of operative time. It’s really 3 procedures: the neck (platysmplasty), one side of the face, and the other. Eight hours to perform a facelift alone is a rather long case. Even when I perform multiple procedures (upper and lower blepharoplasty, browlift, resurfacing, etc.), it is rare to exceed 6 hours.
