Onlay cartilage grafts for depression are usually placed over depressions in the nasal bone. This is over the alar-upper lateral cartilage junction. If this is not where the graft was placed it either slipped out of position or this is scar. Your surgeon is in the best position to make that differentiation. A steroid injection will not get rid of this if it’s cartilage. A fractional ablative laser or a non-ablative laser isn’t likely to affect this whether it’s scar or cartilage because it’s…

When I read your post I didn’t believe it but looking at the photo it does look 6cm wide at the alar base. The desired size is usually equal to the distance between the inner and outer corners of each eye (the intercanthal distance normally is 3.2cm). That would require an alar base reduction of 1.4cm from each nostril rim. That is more than usual so I suspect you will also need some thinning of the nostril rims in order to decrease the resection size to avoid a pinched nostril appearance.

The blepharoplasty incisions in the upper eyelid are in the crease and in the lower lid just below the eyelashes or on the inner surface. Since the tattoos are near the eyelashes they aren’t significant for upper blepharoplasties though they make look wider after surgery. For the lower eyelids if the incision is made on the inner surface the tattoo is irrelevant. If the incision is on the lower eyelid skin it may be okay depending on the width of the tattoo.

Many patients have increased visible blood vessels on their nose skin after rhinoplasty. Without photos or more information it is impossible to say what your problem is. It could be just increased vascularity, rosacea or something else. Most of these will not resolve with hyperbaric oxygen. From the time frame it does not sound like the skin died. If your eyelid resting position changes during the course of the day you need serology blood tests for antibodies against the acetylcholine receptor and MuSK protein, electromyographic muscle tests and/or test for resolution after application of ice before you undergo any surgery because the problem may not be amenable to surgery.
