Will botox for gummy smile lengthen my upper lip at rest?
Botox for gummy smile
Aug 16, 2021
It is possible that the Botox could slightly increase your upper lip length, but we actually inject Botox in this area to do just the opposite (Botox lip flip). This everts the lip and shows more pink lip. To some degree it depends on which part of the muscle you are injecting. To crease a lip flip, I focus my injection closer to the vermillion border (orbicularis oris muscle) to weaken the portion which is responsive for curling the lip inward. When injecting for gummy smile, I usually try…
How do I reduce nostril flaring without decreasing upper teeth exposure?
Nostril flare
Aug 16, 2021
Although I am limited by the photos, it appears that your ala do widen with smile excursion which is normal to some degree. The nose is nicely balanced at rest, but with smile I recognize your concerns. The muscles which cause you to smile act to pull the alar (notril) soft tissues laterally and superiorly. This effect cannot be fully diminished with botox as it would cause some degree of facial paralysis to inject the main smiling muscles (zygomaticus major/minor/risorius). It can be worth…
Gummy smile cause and what are my options if I want to fix it?
Gummy smile cause
Aug 16, 2021
It appears that the primary cause for your gummy smile is hyperactive upper lip elevators, but you do also to have a slightly longer premaxilla which contributes to your gummy smile as well. Well placed botox may help this. This is a good first treatment before consideration of more aggressive surgery like LeForte osteotomies or orthognathic surgery should be considered. In reality your upper lip length does match well with the length of your mandible (chin/jaw) so care should be taken when…
Is PIH a high risk if I have dark skin and am getting tear trough filler with cannula?
PIH and cannula
Aug 16, 2021
It is unlikely that you will develop and significant PIH with the cannula technique. In fact, with the cannula technique there is generally only one insertional hole needed which limits the amount of times the skin would need to be punctured. I treat many people of color with history of PIH and have never experienced any PIH with my undereye cannula technique.
Help! Immune to Botox (and all neuromodulators), verified by 3 separate dermatologists
Glabellar folds and botox intolerance.
Aug 16, 2021
It has been observed that some people develop antibodies or intolerance to neuromodulators. In this case, it is worth a several month hiatus and then reassessment with larger doses. The other option is to weaken the muscles with a endoscopic brow lift. During this procedure, the brow can be lifted and the muscles in the glabella excised/cut. Although there is evidence that muscle function does return, many surgeons anecdotally report that the activity is never as strong as it previously was….
What is this fold on my face and what could help?
Facial fold
Aug 16, 2021
Yes this appears to be a Marionette line from the photo. It is related to laxity in the soft tissue support and loss of bone/soft tissue volume near the mandible. There can be several approaches to improve this fold depending on your goals and desires based on least to more aggressive intervention tolerance. The least invasive treatment would be well placed filler to support the lower face. This should not be overdone as it can cause too much fullness/facial heaviness. The other, more…
Overall asymmetry – are these connected?
Overall asymmetry
Aug 16, 2021
It is normal to have some degree of facial asymmetry. It is related to how we develop. Those asymmetries can be related to differences in bone development, soft tissue size, shape, decent as well as environmental exposures like asymmetric pressure and sunlight. Specific cause of your facial asymmetry cannot be acertained with the photo alone, but in person a physican be be able to give you more details. Given your photo, the clicking in the right jaw may be associated with TMJ which can be…
