I’m glad to hear that the finasteride is helping. Note that it can indeed be confusing if you just leave it to the doctor to tell you what you need. I prefer to do some computer simulations for my patients to help give them a realistic idea of what I think can be done with their nose with rhinoplasty, especially after assessing skin thickness. That way, we can also make sure that the patient and I agree on the overall size, width, rotation, projection, etc. of the nose beforehand. Ask your surgeon for specific recommendations, but generally speaking the recovery from a revision rhinoplasty should be similar to the first surgery. The exception would be if rib grafts were anticipated, which adds the nuisance of some chest discomfort and additional incision care. Rhinoplasty can address all of those concerns. Depending on your specific anatomy, it may be possible to widen the nose surgically with spreader grafts, onlay grafts, and other techniques. However, if the desired correction is minor enough, a non-surgical approach with injectable fillers may be a good option. “Diagnosis before treatment,” as we say. You should probably see an ear, NOSE, and throat surgeon about this soon. He/she can better determine what it might be first (probably an inflamed or infected inclusion cyst). It seems unlikely that the tip will improve over time, but it’s still important to wait at least a year from the revision before proceeding with yet another operation. From the photos, it does look like the upper blepharoplasty was under-corrected. I can only guess at the reasons for that. Perhaps the surgeon was overly cautious and hesitant to take the amount of tissue necessary. There also appears to be some degree of brow ptosis (brow droop) as well. As you’ve correctly pointed out, thick skin can be a major limiting factor in what can be done with rhinoplasty. It’s hard to tell from the photos how thick the skin is, but during an in-person consultation, the surgeon should specifically assess your nasal skin thickness as part of the exam (if the surgeon doesn’t, run!). If it’s bothering your breathing enough, it’s quite worthwhile to have your deviated septum repaired. Note that from the photo you appear to have what’s called a caudal end deformity of the septum. This can require a slightly different approach than a typical septoplasty operation (i.e., usually creating a caudal end strut graft to avoid tip collapse or columellar retraction). That approach allows for the full range of techniques to reshape the nose to the patient’s preferences. Reducing the overall size of the nose, eliminating the bump, refining the tip, etc. are all possible. Find a doctor whose primary focus in plastic surgery is rhinoplasty.

The photos suggest you have eyelid ptosis, worse on your right than your left. This is best treated surgically by an ophthalmologist (eye doctor) who specializes in oculoplastic surgery. In experienced hands, it’s generally a fairly straightforward fix.

Thanks for the good pictures of your nose! Yes, these are problems that can be addressed with rhinoplasty. I tend to prefer the open approach for this since it allows better control of the tip, particularly when the tip needs to be de-projected and the nostrils shortened. These are, however, somewhat more complicated aspects of rhinoplasty, so be sure to find someone whose practice is primarily facial surgery with a lot of rhinoplasty experience.

If you intend to keep your ears looking good, I wouldn’t recommend it unless you intend to wear good quality sparring headgear that protects ears at all times when you’re in the ring. As you seem to suspect, you’re likely to damage your surgical result and given your tendency to form keloids additional correction would be hazardous.
