You have a skin envelope that will allow you to accept an implant that will give you the desired result.  Implant selection will need to be made after a thorough clinical exam. Your PS will give you specific recommendations on style and size of implants after your consult. 
Will UHP implants leave me with a big gap and too fake looking breast?

Implant selection is based in numerous factors including base width, tissue coverage , and patient preference. I would avoid the UHP implant as the ratio of width to projection is extreme, and in my opinion would lead to an unnatural result. The width of the UHP implant is narrow and may exaggerate the gap between the breasts. 
5’8 130lbs having trouble picking implant size, would love some advice?

Your breast width of 12.3 on a 5’8″ frame seems quite narrow. If that is the measurement,  I would lean towards the smaller of the two sizes you mentioned. Even on a tall frame you should aim to keep the width of the implant at or narrower than your breast width. 
Would a Tummy tuck help me (diastisis recti but no extra skin)?

With four children your rectus muscles will need repair. Once this is done, your rounded stomach will flatten and you will be left with redundant skin. Therefore, a full tummy tuck would be done in combination with the muscle repair. Seek the opinion of a local qualified Plastic Surgeon and good luck !

Cost varies dramatically between surgeons and cities. There are number out factors to consider, but in general, surgeons within be city centre will charge more given higher operating costs. Surgeons with greater experience may charge more, and an overnight fee will likely be applied given the extent of the procedure.
