Botox or Dysport would be a great place to start for the brow furrow (i.e. "11's"). These agents smooth the surface of the skin and the look is often youthful and very natural. Bruising around the mouth and nose area is quite common after injections. I always tell my patients to avoid aspirin, alcohol, NSAIDs, herbals (G's: Ginkgo, Garlic, Ginseng) at least one week before treatment. If a bruise develops, it usually takes about a week to resolve. The best advice is to find a concealer, such as Dermablend, to hide the bruise during this time. Hang in there! Miriam Hanson MD.

Tretinoin cream is a great medication for acne. It should be used correctly to minimize the common side effects which include redness, dryness and scaling.
For application: Gently clean your face with a mild or soapless cleanser. Wait 20-30 minutes before applying this medication. Apply a pea-sized amount of cream usually once a day before bedtime. If your skin is sensitive, start once or twice a week and increase the medication as tolerated.

This is a very common response to Retin-A creams. I usually tell patients to discontinue the medication for one week and then restart it 2x/week and increase as tolerated. Use a gentle moisturizing non-comedogenic (non-acne forming) "cream" 2-3x/day during this build-up phase. The moisturizer will help keep the skin hydrated and these side effects will diminish.

Yes, you can discontinue and restart for a short period of time while on vacation to reduce sun sensitivity. However, you may get an acne flare. I usually recommend to my patients that we lower the dose to 10 mg and then increase back to the full dose when they return. Either way, you should be "Sun Smart" and minimize sun exposure while on vacation. Accutane has a serum half life of 21 hours but can stay in the system for several days to weeks depending on its break down by the liver.
