For sure, both skin and breast tissue can be removed in a revision surgery. If the nipple position is okay, then taking skin and breast as a wedge using the same vertical scar would be possible. It’s likely that this would correct any lower pole heaviness and give a better shape.

A breast can be made as small as you want it to be. The main limitation however is the ability to safely keep the relocated nipple alive. In extreme cases it is possible to remove the nipple as a graft and the breast can be made any size and the nipple is sutured back on. You are slim and your breasts are proportionally very large and sit very low on your chest. There may be an element of tuberous breast deformity as the fold under the breast looks quite high from your photos. You would do best from a reduction and reshaping with an anchor shaped scar to bring the nipple and breast up on the chest and reduce some of the weight. This would be a really positive change for you! There are rebates from Medicare or your health fund for this type of surgery.

This is a relatively common area where an extension of the breast tissue can exist. If it is mostly soft fatty breast tissue it will respond well to liposuction. If there is a firmer glandular component it would need to be ‘excised’ which would mean an access incision and scar. It looks like you have a scar in the armpit. The biggest risk in redo breast reduction is the blood supply to the nipple being compromised enough to kill it. It is thankfully a rare complication and if caution is taken during the surgery it will be avoided. If there is any doubt that this may be the case the nipple can be removed and replaced as a graft which increases its likelihood of survival. You are absolutely eligible for a breast reduction! We still battle with the public misconception that breast reduction is a cosmetic procedure. This is simply not true. For all the same reasons you point out and more. Neck and shoulder pain, shoulder grooves from bra straps, rashes under the breasts, fungal infections to mention a few. Let alone difficulty with exercise, buying clothes and bras.

It is unlikely that fat dissolving injections or chin lipo would help your protruding chin. Bringing the chin forwards or back is better addressed by changing the shape of the bone. Reducing how much the front of the jaw sits out is likely your best option. Depending on how much set back you want, it can be as simple as surgically sculpting the front of the chin bone.
