Based on the photos shown, a deep plane facelift with deep neck lift performed by an experienced Facial Plastic Surgeon will provide great benefit to your lower face and neck. The obliquity in the neck is related to laxity of the platysma muscle (which starts in the lower face), subplatysmal fat, and possibly ptotic digastric muscles. Liposuction alone and/or energy devices will not be comprehensive in addressing this problem.

Festoons are a difficult problem to treat, explaining why there are multiple approaches to address them. I have seen success with an extended lower lid subiliary blepharoplasty and SOOF lift. This is a pre-periosteal approach. My rational: the periosteum is a very deep, thick, inelastic layer while festoons are a more superficial soft tissue finding between eyelid and cheek. This is a scenario where you choose the Surgeon and his/her results/experience/judgment over the specific approach.

Physiological age and chronologic age can be different in Surgeons; I have seen some Surgeons in their 50's in terrible shape and much older Surgeons at their prime. We have all seen Surgeons perform very well into their late 70's, and it is possible that your Surgeon is in great shape. This isn't an easily answered question by outsiders and best to discuss with the Surgeon you are seeing.

Regarding smoking 11 days before your surgery, "...if it seems like a bad idea, it probably is". Remember that it is your nose that is being operated on, and it will be your result. I would definitely notify your Surgeon and not smoke any more at this point. It is certainly sub-optimal situation and will rely on the judgment of your Surgeon.

Great question. Although a consultation is necessary to give a complete answer, the photos certainly show indications that a facelift would address. There is evidence of descent of mid-facial soft tissues jowling, and obliquity in the neck. The brows are relatively "heavy" in terms of facial soft tissues. An experienced Botox injector can help achieve symmetry on a temporary basis, while an endoscopic brow lift by an experienced Facial Plastic Surgeon will do so on a permanent basis. Threadlifts are expensive and temporary and often ineffective. Take a nice vacation before doing threadlifts!

In terms of what you are having performed and lag time to the camera, I would suggest an 8 week window minimum, 12 weeks to be safe. Since this relates intimately to your profession, please do your research and choose your Surgeon wisely. I highly recommend Dominic Bray FRCSEd(ORL-HNS) on your side of the pond.
