Easiest way to see if the new drops work is to try them.
Question to you: are you bothered by the ptosis from a functional standpoint, or do you just not like that the right and left eye look different? Because if your concerns are asymmetry only, forget about the ptosis and just remove a little tissue from the right upper lid. It will help your symmetry significantly and is easy.

I am sorry that you had a bad experience. I don’t think it means that you had a bad surgeon, however. I find that the phenylephrine test is most useful, for me, as a teaching tool when discussing surgery with patients. I am not convinced that the quantitative outcome of the PE test really matters that much.

Hi rhys03,Your eyes look great, but I see that there are some differences between right and left. The eyeshadow space on the left is a little larger than that on the right, and there is a small extra fold on the left.
The differences in eyeshadow space are due to slight brow asymmetry. The easiest way to compensate is just to remove a little skin on the right upper lid, to unmask the eyeshadow space more on that side.

And then it occurs to me: if I have a hard time telling THAT, then perhaps you don’t have much to gain from further surgery!

Hi there,
I see your cause of asymmetry. There is no true excess of skin on either side, but for some reason, the skin on the right upper lid folds out and the skin on the left upper lid folds in. The simplest way to improve symmetry is to simply remove some tissue from the right upper lid. It is a 15 minute office procedure and a home run, with minimal down time.

I think that you have had very conservative surgery. While you do have ptosis, repair of it needs to be done in a thoughtful manner, because as you lift the eyelids higher, you will naturally see less eyeshadow space. A conservative ptosis repair with a more aggressive blepharoplasty will get you to the right end point. I agree that a brow lift is not something relevant for you at this point.

You will get 90% of your symmetry with a conservative right upper lid blepharoplasty. It will take fifteen minutes, is done in the office, is easy, and you won't look back. Nobody notices a tiny bit of brow asymmetry. They notice the asymmetry of the eyeshadow space. Just remove a small amount of skin on the right upper lid and you will be good.

Hi there,
My understanding of your complaint is that you see more of the right upper eyelid (eyeshadow space). This is because your right eyebrow is a little higher than your left, and it pulls the skin fold up with it, showing off more of the eyeshadow space. There are two solutions:
1. Botox to lower right brow a few times per year
2. Left upper lid blepharoplasty to remove some skin, making the lid eyeshadow space symmetric. No one really notices a small difference in brow height, but...

I would not inject filler. I would remove the lower lid fat compartment. It would be a fifteen minute surgery with lasting results.
It is important for you to see an oculoplastic surgeon to make sure that there is no eye socket problem that is causing the fat to protrude anteriorly on that side. Probably not, but worth considering.

Hi,
I think your eyelid looks very good, foremost. There is some puffiness. As a rule of thumb, whenever there is puffiness after filler, the cause is the filler. It doesn't matter what a scan shows. It's the filler. I really would not entertain otherwise unless you inject multiple tissue layers with hyaluronidase and it is still swollen. And if it still swollen after that, I would... inject in multiple layers again.
You might be disappointed after dissolution of filler, because it may leave...

Your eyes look lovely and the last thing you should do is surgery! I would advise a small amount of injectable filler to the lower lower eyelid. You will do quite well with that, although you may repeat it in a year or two.

Hi,
It is totally reasonable to perform right upper blepharoplasty only. That is a 10 minute office procedure in your case with minimal downtime. It will make a significant improvement. However, you have a risk of a redundant skin fold on the right side (you have two folds already), and if you wanted the have the best symmetry, you would elect to have both sides done so that the right sided surgery could be a little more aggressive, capturing the second fold in the excision.

A few observations: you have excellent brow position and a depressed scar at the outer left brow. You have some mild hooding over the lid.
Correction would be a straightforward office surgery: upper blepharoplasty, combined with a scar revision of left brow. The scar revision would leave you with a new scar, but it would be thin, not depressed, and would heal much better than the current scar.

I don't think there needs to be any overthinking on this. There is a touch of extra fat in the lright ower lid medially. This can be removed in 15 minutes with an office procedure. I don't think you should waste time with filler if you are happy with the left side and want the right side to look like the left. There is no incision externally.
