Statement: Dr. Jared Christophel is a lifelong Virginian. He earned his Bachelor of Science in chemistry from the College of William and Mary. He received his medical school education at the University of Virginia and stayed to complete an internship in general surgery followed by four years of postgraduate residency training in otolaryngology–head and neck surgery. After residency, he completed a one-year fellowship in facial plastic and reconstructive surgery under Dr. Peter Hilger at the University of Minnesota.

Dr. Christophel returned to the University of Virginia after completing his training and worked as a member of the faculty for 10 years before advancing to a tenured position in the Department of Otolaryngology–Head and Neck Surgery, Division of Facial Plastic and Reconstructive Surgery. Dr. Christophel is a senior examiner for the American Board of Otolaryngology–Head & Neck Surgery. He has lectured nationally and internationally on his techniques for rhinoplasty and facial reconstruction.

He enjoys offering his patients the full spectrum of otolaryngology care and has a special interest in facial plastic and reconstructive surgery. It is very tough to tell based on your photos, but you likely have an underlying deviation to the septum that is causing your left tip cartilage to look more prominent. See a surgeon experienced in rhinoplasty for a thorough exam and consultation.
Right nostril completely blocked 11 days post septo-rhinoplasty. Any suggestions?

You can have significant swelling after surgery that can obstruct your breathing. Follow up closely with your surgeon.
Can hump removal and mid vault issues be dealt with in the same Rhinoplasty procedu

When dealing with a dorsal hump the surgeon will usually have to deal with some component of the middle vault. If you are having collapse issues then the rebuild of the middle vault will have to be done with the collapse in mind. This is usually best done through an external approach. Good luck.
What is the Prevalent Method for Costal Cartilage Harvest?

Leaving the perichondrium may allow some fibrosis to occur in the region, but the cartilage generally doesn’t grow back. Many surgeons will use the perichondrium as a graft to soften the nose and prevent any lumps/bumps from showing through the skin.

If you are able to close your eye reliably, Gold weight probably not necessary. Tightening your lower lid may be helpful.

An Otolaryngologist – Head & Neck Surgeon would be a great place to start. Many of them have small endoscopes that can be used to examine the inside of your nose.
What surgery will I need to repair my nose?
It depends on the cause and extent of the perforation

First of all, progressive causes of the perforation should be ruled out. After those are ruled out and the perforation has stabilized in size, the reconstruction will depend on the extent of structural loss. Your repair could consist of anything from onlay camoflauge grafts to rib graft reconstruction (more likely).

The major concern with operating on the nose of children is that the “growth centers” of the nose would be disrupted, and the nasal growth afterwards would be stunted. However, sometimes the area of obstruction can be individually addressed with less risk to the growth center, or if your child is almost full size it may not be much of an issue.

Depending on how much work was done on your septum, it is possible to lose tip support. Multiple things can happen anywhere from the tip cartilages migrating down the septum, to your septum itself becoming displaced posteriorly for multiple reasons. I would have an evaluation by a surgeon who performs a lot of rhinoplasties.

Your symptoms could be very normal post op symptoms, but would be good to have him/her examine the inside of your nose. I have seen nasal surgery cause migraine-like symptoms in patients which it sounds like you may have.

The best plan depends on a meticulous exam of the inside of your nose (maybe even with an endoscope) in a serial fashion to see if the scar is evolving. If it is just a simple scar band bridging the nasal cavity (a synechial band) I would fix it sooner than later because continued scar contraction could possibly cause the septum to deviate towards the affected side. Regarding the nasal obstruction, it is still way to early to say the congestion is to persistently deviated septum or displaced nasal bones. Would give congestion 6 weeks prior to making that conclusion. If he does have slight deformity or some obstruction, may need corrective surgery once he is done growing. Typically don’t want to perform “open” surgery on child’s nose as it may disrupt the growth centers of the nose.

Treatment of delayed enopthalmos is possible, but depends on the anatomic cause and is not without risks. Given the title of your username, I’m guessing you also had some trauma to your zygoma at the time? You could have something as simple as some nasal vestibulitis (inflammation of the hair follicles just inside the nose) or something more complex like an exposed cartilage graft on the inside of the nose that is draining/crusting. It all depends on what you had done at surgery as to what the possibilities are. In summary, go to see your surgeon and get your nose examined thoroughly.
