Rhinoplasty after nasal trauma
Nov 14, 2023

Hi!
I commonly see patients that find themselves in a similar situation to yours. Nasal trauma can results in the nasal bridge being deviated/crooked from the front and this is a common indication for getting nasal surgery. In many cases, people can develop problems breathing after the injury and this can typically be addressed at the time of surgery as well. Usually I recommend waiting 3-6 months after the injury for definitive repair and if you plan on continuing to do activities where…

Is it safe to have a rhinoplasty done at the age of 48?
Rhinoplasty Age 48

Hi,
While the structural framework of the nose does change with time, I have had plenty of patients get nasal surgery at your age. I would recommend a full assessment with evaluation of the internal and external aspects of your nose by someone that does a lot of nasal surgery. As we age, there are additional technical considerations but age alone should not prevent you from having nasal surgery. Hope that helps!

8 months post rhinoplasty – columella feels quite stiff
Columella
Nov 14, 2023
Nov 14, 2023
Hi!
The horizontal line on the upper lip is where a muscle inserts (depressor septi nasi muscle) that connects the nasal septum to the upper lip and can lower the position of the nasal tip with smiling. The stiffness of your columella is not unusual after nasal surgery and especially so if you had a caudal septal extension graft placed to stabilize the position of your tip. This will soften with time but I’ve had patients report that their tip and columella feel stiff for 18-24 months after…

Is it unreasonable to return to work the next day after rhinoplasty? I work from home
Rhinoplasty Recovery

Hi!
I have had patients go back to work as early as 1-2 days after an open rhinoplasty. In my experience the main things that would limit your ability to go back to work would be pain and swelling around the eyes. If you don’t have to be in front of a camera or clients, then it’s not unreasonable to think you could go back to work after a few days. hope that helps!

What can be done for my alar retraction?
Alar Retraction
Nov 14, 2023
Nov 14, 2023
Hi,
yes, it does look like you have retraction on the right side. This can happen with reductive rhinoplasty techniques when the lateral crus (outer part) of the tip cartilage is removed leading to scar tissue and upward displacement of the alar margin. A composite graft can lead to improvement in the symmetry of the nostril margin. I’ve had less success with filler and urge caution to patients who are considering filler after a nasal surgery as the vascular anatomy can be affected by the…

Is 61 years old too old for a rhinoplasty?
Rhinoplasty at any age
Nov 14, 2023
Nov 14, 2023
Hi!
I’ve done many nasal surgeries and I’ve found that so long as expectations are realistic and you feel comfortable with your surgeon that age 61 is definitely not too old to get a rhinoplasty. As you mentioned, noses tend to droop as we get older and this is due to a number of factors: primarily the effect of gravity and the loss of inherent strength of the ligaments and cartilage in our noses as we get older. These are discussion points at the time of consultation but shouldn’t prevent…

Hi!
As you may know, the tongue-in-groove technique typically involves moving the tip cartilages upwards either onto your septum or onto a central graft. Without seeing the operative report, it’s difficult to say what exactly was done but given the length of time since your initial surgery, I find it to be unlikely that simply removing sutures would fully address the retracted columella. In my experience, and depending on the scarring, this would likely require the tip cartilages to be…
