Is my chalazion too small to be removed? Am I stuck with this for the rest of my life?
You don't have to be stuck with this forever
Feb 10, 2020
Thanks for your photos and questions. A chalazion is a condition on the spectrum of eyelid gland dysfunction, or a problem that can occur as a result of certain glands in the eyelid becoming "clogged." Sometimes the chalazion can form a dense capsule and become more like a cyst. In these cases, it can take many months to resolve. The size does not matter, although it is more common for these types of cystic chalazia to be larger. The treatment can include warm compresses, steroid i...
Could something be done to get rid of the red line is I seek additional plastic surgery?
Cosmetic concerns after Hughes reconstruction
Feb 10, 2020
Thank you for your photos and questions. You have undergone one of the most advanced reconstruction techniques, which is reserved for cases where a large portion of the lower eyelid is affected by cancer. Overall, your eyelid appears to have healed well with regard to position and function based on the limited information that photos can provide. Because tissue is borrowed and transferred from the upper lid to perform the reconstruction, it is not uncommon to have some inflamed a...
32yr old female with under eye insecurities. What can I do about this?
Undereye insecurities
Feb 10, 2020
Thank you for your photos and questions. It sounds like your main concern is the appearance of the lower eyelids. Based on the photos, you do have some "undereye bags" which is typically caused by prolapse of the orbital fat pads. It is not uncommon to start to notice this at a young age, especially if prominent eye bags run in the family. These could be readily addressed with transconjunctival lower eyelid blepharoplasty with or without fat repositioning. It is hard to appreciate the ve...
I’m going to be having cataract surgery later this year. I want to have eye bags removed at some point
Which surgery to have first
Feb 10, 2020
Thank you for your question. There is no single correct answer to this, and it really comes down to your priorities. However, if your cataract surgeon has deemed you a surgical candidate, this would indicate that your cataracts are impacting your vision enough to justify surgery. Therefore, it would be reasonable to pursue this first and reap the benefit of functional improvement in vision, and defer the cosmetic improvement in lower eyelid appearance for later. As the other doctors have p...
Do I need upper and lower eye surgery?
Choosing an eyelid procedure
Feb 10, 2020
Thank you for your photos and questions. With regard to upper eyelid blepharoplasty, it would probably not be worth it for you at this point, since you do not have significant skin redundancy (dermatochalasis) of the upper lids. Should you decide to pursue this in the future, any fat sculpting should be done conservatively as you are at higher risk of developing a hollowed out appearance especially on the left side. With regard to lower eyelid blepharoplasty with or without fat transfer, a...
Follow up question: Eyelid not folding back, but not sure if it’s Ptosis, as no drooping. Advice? (pho
Brow asymmetry
Feb 10, 2020
Thank you for your photos and question. Based on the photos provided, you do appear to have a mild to moderate degree of ptosis, however it is largely symmetric and is not necessarily a "problem" that needs "fixing". The difference in how the skin of your upper eyelids is folding is most likely coming from brow asymmetry. In the third picture, your right eyebrow (photo left) is higher than your left eyebrow. A higher positioned brow will pull up on the upper eyelid skin and make it look l...
Day +8 blepharoplasty of 4 eyelids. What happened and what to do? My upper eyelid cannot fully open up
Early postoperative asymmetry
Jan 26, 2020
Thank you for your photos and questions. At this stage in the postoperative period, you are still within the span of time during which the swelling and bruising can be asymmetric. The photos do not clearly show worrisome eyelid malposition, although one side does look mildly more swollen than the other. Even though you had symmetric surgery, it is normal for your body to heal asymmetrically. Although our body parts can look symmetric on the outside, the capillary...
Is transconjunctival blepharoplasty with fat relocation and no skin removal a safe approach?
Transconjunctival blepharoplasty
Jan 26, 2020
Thank you for your questions. Having familiarized yourself with possible complications shows that you are invested in getting a good result and that you prioritize safety. The transconjunctival technique is favored as the risks of ectropion and retraction (scleral show) are lower than if the same procedure is done transcutaneously or with a skin pinch. Scarring of the skin is not an issue if this is done transconjunctivally, and scarring of the conjunctiva is rare in...
How is globe-lid disjunction treated?
Globe-lid disjunction
Jan 26, 2020
Thank you for your photo and question. Based on the photo, you have a fairly uncommon condition called euryblepharon as well as lower eyelid laxity contributing to the scleral show (white of the eye that can be seen between the iris and the lower eyelashes). Treatment of a situation like this is highly individualized as many factors come into play including what the rest of your face look like, how much support you eyelids get from your cheeks, and any medical conditions that may also...
I'm noticing dynamic wrinkles under my left eye only. What is the best treatment?
Asymmetric dynamic lines
Jan 26, 2020
Thank you for your photos and questions. The hollowness looks like a mild to moderate tear trough, although it is hard to appreciate the extent because of the diffuse lighting. This tends to be an facial feature that we are born with and is just part of some people's facial anatomy. It can respond well to dermal fillers in select patients. As for the wrinkles around your eyes when you smile, this is a result of the orbicularis muscle contracting. Why it shows up...
1 year post op eyelid surgery, upper eyelid on one eye is smaller. Can I ask to fix it?
Eyelid and eyebrow interplay
Jan 26, 2020
Thank you for your question and photo. It does appear that your left eyebrow (photo right) is riding higher than your other brow, which is giving your upper eyelid an asymmetric appearance. There are many ways to adjust brow position ranging from neuromodulators (Botox, etc.) to additional surgery. The best initial approach to help you attain the result you want should come from a discussion and exam with your surgeon.
Why do many celebrities that have had their eyes ‘done’ seem to have smaller eyes afterwards?
Smaller eyes after blepharoplasty
Jan 26, 2020
Thank you for your questions. Without a specific celebrity in mind plus before and after photos, it is hard to comment on what you are noticing. However, some amount of swelling is a normal part of healing, which, after an upper eyelid blepharoplasty for instance, can for a few weeks to months make the eyes look smaller or less open. It is unusual for swelling to cause frank eyelid ptosis, and may be an indication of a problem if it does, but there is a possibility that what you're...
Why is a fat transfer recommended for doing my lower blepharoplasty? And what will happen if I don’t i
Variations on lower eyelid blepharoplasty
Jan 26, 2020
Thank you for your question. There are as many ways to do a lower eyelid blepharoplasty as there are surgeons that do the procedure. The specific approach with regard to fat manipulation tends to vary by which modification of the technique has provided the best results in a given surgeon's hands. Fat transfer and fat transposition are all different ways of using fat that would normally be removed to fill some of the surrounding hollows. I reserve fat transposition for my...
Will a Filler or Belpharoplasty Fix my Under Eye Bags?
Undereye bags
Jan 25, 2020
Thank you for your photo and questions. It seems that you may have multiple components that are contributing. The crepe-like appearance is likely to do well with laser treatment, as you mentioned you are considering. If you feel like there are lines around your eyes that accentuate when you smile, you may see some benefit from neuromodulators (Botox, etc) as well. The bags that you have under your eyes are fairly well defined in the photo, which suggests they are probably related...
What can I do about my horrible under eye circles and bags?
Dynamic lines below the eyes
Jan 25, 2020
Thanks for your photo and questions. I assume that in the photo you are smiling, as the muscle around your eyes (the orbicularis oculi muscle) is activated. There is some asymmetry, with your right eye (photo left) having more fullness in the lower eyelid than the left. If this is only apparent when you smile, neuromodulators (such as Botox) may be an excellent option for you as these can help the muscle not contract as tightly, reducing its prominent appearance....
