What's the chance of having a successful revision rhinoplasty with a piece of cartilage hanging
Revision rhinoplasty can be successful if done right
Feb 21, 2023
Hi,
Revision rhinoplasty is nearly always an option if done right and in the right patient. What I mean by that is that many things can be corrected, but may need rib cartilage for example to rebuild the nose. If the patient is motivated and willing to do this type of procedure then it can absolutely work and have great results. It is very rare for the nose itself to be so unhealthy that revision is too risky, and in those patients for which there is moderate concern for the health of the...
What can I do to reduce scar tissue after an open rhinoplasty?
Kenalog vs. revision
Jan 12, 2023
Hi,
It would be worthwhile to know how long it has been since surgery. In the initial healing period scar tissue forms and there is a significant interplay between scar formation and transient swelling. We deal with the swelling and early scar tissue primarily with kenalog injections, but also with things such as taping, nasal massage/exercise, diet modification, behavior modification, medical management of infection. If surgery was a while ago and the initial healing/scar phase is...
How risky is a third rhinoplasty?
Revision rhinoplasty can be quite safe and successful
Jul 7, 2022
Hi,
I generally think that if you are healthy and the skin and soft tissues of your nose are healthy, then it is reasonable to undertake a revision rhinoplasty, even if there was more than one surgery before. You do have to allow for sufficient time for the nose to heal; if your last surgery was in 2019 then that should be plenty of time.
However, I would say that these surgeries usually require a different approach, and frequently that means using some other source of cartilage like ear...
Is it possible to trim pointier side of tip in a small revision?
I would avoid trimming the tip cartilages
Jun 16, 2022
In cases where there is a "pointiness" on one side of the tip associated with a different nostril shape, I would guess that the underlying issue may be an asymmetry of the tip cartilages. This theory is supported by the fact that if you push down on the pointy side it helps fix the nostril issue. One slight caveat is that at 10 months out there is still some swelling reduction that needs to occur, although it is a decent time point to make an assessment.
However, I want to make the point...
What procedure would be best for nasal valve collapse?
Tough question!
Jun 16, 2022
Your question is one of the most important questions in the nasal surgery process - how is the patient supposed to know which plan is the best when two surgeons propose what seems like very different options? In my opinion, it comes down to the consultation. I'll explain what I mean.
Your consultation is the critical time where you can learn about rhinoplasty and get your specific questions answered. I personally feel that if the surgeon does not really spend the time to answer your...
I believe I have a deviated nasal septum, what surgery do I need?
Caudal septal deviation is different than regular septal deviation
May 26, 2022
Hi!
This is a great question, and the photos you provided are excellent and show the issue clearly.
Your septum is indeed deviated. However, the important thing to point out about your particular septal issue is that it is a specific part of the septum that is deviated, which changes the way a surgical correction is done. As you can clearly see from the photos, the very front of the septum is what is twisted and deviated. This part of the septum is the "caudal" septum, and it is actually the...
Revision rhinoplasty - Prone to keloid. Is this common practice?
Open vs. Closed rhinoplasty; columellar scarring
May 26, 2022
Open rhinoplasty is a powerful approach that many surgeons prefer to get the outcomes they want, especially when it comes to the nasal tip. I have performed hundreds of these operations and seen many more, and have not seen a patient with a keloid in that location. Keloid formation depends not just on the individual's tendency to from keloid scars, but also the location of the scar itself. Further, scarring can be managed to some extent if followed closely after surgery. You will need to...
