I would say very few patients feel a true choking sensation after a facelift and platysmaplasty. There is a definite tight feeling of the neck but very few people in my experience feel like they are choking. The area of the lower face and neck can certainly feel tight and numb at the same time which can be an odd feeling. I find this is a short term concern and generally resolves within a few days to weeks for my patients.

The lower eyelid fat pockets tend to be diet resistant areas. Additional weight loss should not significantly affect the fat pockets but can impact the surrounding tissues in this region. In any procedure in plastic surgery it is not necessary but probably best to choose a surgeon who specializes in the specific procedure you desire. I recommend you review before and after photos of your surgeon’s work during your consultation and assure she or he has a clear understanding of your goals.

There is not an absolute best age for a facelift. An individuals genes, exposures, and medical conditions can all contribute to facial aging. Taking into account your photos and age it appears you are a little premature for invasive surgery. I would recommend trying non-invasive skin tightening treatments such as Ultherapy or other skin tightening devices in addition to fillers at this point.

Antibiotics are not necessary after a facelift. The risk of infection is very low following this type of surgery and studies have not demonstrated a clear advantage. In my practice that focuses on facial rejuvenation we give one dose of antibiotics through the IV prior to facelift surgery, but no other antibiotics are necessary.
