There are a few issues going on with your nasal dorsum. There are also less invasive ways to correct these issues and more involved ways to correct them. In my practice, I would choose to pursue the more involved approaches so that the nose will look like you had originally hoped that it would and also so that our result will be permanent. I would discuss your concerns with your surgeon and they should be able to help you determine the best approach that will fit your desires.

It appears that you developed either a pressure ulcer or ischemia following the deep plane facelift. It may have been due to dissection too close to the skin and not leaving the skin flap thick enough, or perhaps a tight pressure dressing applied following the procedure. The other issue is the immediate postoperative appearance of the submental incision which appears to be infected and the neck may have succumbed to the same infection. So you are actually giving two conflicting procedures. I suspect that your surgeons intentions are to perform a transconjunctival lower eyelid bleph with a skin pinch at the end. The risks of ectropion in these cases is less than 1% unless there is significant lower eyelid laxity, If the surgeon is cutting through the muscle then they are not performing a skin pinch, but a true lower lid skin muscle flap.

You are correct. There is a gap between cartilage grafts that were placed along your nasal dorsum during your revision surgery. The easiest way to remedy that issue would be to place a cartilage, temporalis fascia, or both along the irregularity without having to go through a complete revision surgery that might create other issues, It would not be nearly as invasive as your prior surgical procedures.

You would certainly be a good candidate for non surgical rhinoplasty. Your will likely have a fairly high but straight dorsum. Computer imaging of your photos with a virtual or in person consultation will allow you to see what you can expect from a non surgical rhinoplasty. Incidentally, you are also an excellent surgical rhinoplasty candidate and the results would be quite appealing.

Your nose looks pretty good. If you were to perform aesthetic measurements on the amount of columella show you now have I think you would find that you fall into the norm. I would strongly suggest leaving your nose alone.

Botox injections will be safe and effective with your particular prothrombin mutation. You have no more risk for clotting with Botox injections than you would any other day of the week. More invasive surgical procedures need to be approached more cautiously if you are entertaining these in the future.

Congratulations on the weight loss. I am sure you are looking and feeling great. I would not suggest pursuing threat lifting to address your neck. It will be expensive and not at all effective, nor will the results last. You are a surgical candidate and when you can get up the nerve (understandable) and finances for a face and neck lift, find a good surgeon and you will not regret your decision.
