The subciliary approach to lower lid surgery can increase the risk of lower lid retraction if too much skin is removed. This pulling down of the eyelid is different than ectropion. Lower lid retraction generally needs surgery to repair it. In my hands, I perform all my surgeries via a transconjunctival approach.

Ptosis refers to the position of the lid without regard to skin excess. Often when one lid is too low (your left upper lid) the other lid may need to be raised at the same time due to Herring’s law. You are a great candidate for lower lid blepharoplasty. This surgery utilizes an incision on the back of the lid to access your natural lower lid fat that causes the “bag.” This fat is then moved to fill the tear trough and midface to give you a natural transition from the lid to cheek. The skin is either tightened with CO2 laser or in some cases a skin pinch is also performed. Best to see an eyelid expert.

Bumps on the end of the blepharoplasty incision can happen. If the injections (likely Kenalog/5FU) do not improve it then excision is best. Typically, if the bump has not improved after 3 months then I offer excision.

In my hands, the best way to fill the tear trough is with lower lid blepharoplasty (fat transposition) and potentially additional fat grafting to the midface. This surgery takes your natural fat and moves it to fill the tear trough. Additional fat may be needed in the midface which is done with fat grafting. Best to see an eyelid expert.

Nicotine is a not a contraindication for lower lid surgery. Nicotine products do slow the healing process to some degree but you would not put you at significant risk for lower blepharoplasty. Best to talk to your surgeon about all mediations prior to surgery.

It appears you have microptosis of the left upper lid. Often when one eyelid is low the brow tries to compensate and pulls up (frontalis over action). This effectively makes the eyelid look longer compared to the other side. This can be fixed by raising the lid to improve symmetry.

The Muller’s muscle can be manipulated to elevate or lower the eyelid.
Typically, a Mullerectomy (or internal ptosis repair) is used to raise a droopy upper lid. During this procedure the upper lid everted, conjunctiva and Muller’s muscle is excised, and sutures are then placed. This effectively tightens Muller’s muscle and raises the lid.
For upper lid retraction repair, Muller’s muscle is disinserted from the tarsus to help lower lid.

The symmetry of the eyes depends upon the eyelid height, brow position, and soft tissue around the eyes. It appears you have a slightly droopy left upper lid with a corresponding lifting of the left brow (frontalis over action or compensation). This makes the eyelid platform longer compared to the right side. Surgery can improve the symmetry. Best to see an eyelid expert.
