Yes there does appear to be some component of asymmetry to either your chin or jaw. Without an examination of your bite it’s hard to tell which: however, with your TMJ issues, it could more likely be the jaw or mandible in this case. You are still very early post injury. Based on the photos, you also had a left angle fracture. The left angle appears to still be a bit swollen possibly from the injury itself. This may just take time. I wouldn’t be to hasty to do anything just yet. Give this at least 6-8 months to heal before you make any decisions. BSSO surgery affects the posterior mandibular body not the angles as VRO can. As long as the surgery is performed correctly, your angles should be unchanged. Your chin position and dental asymmetry as well as any overjet/reverse overjet should be corrected. These are important questions to discuss with your surgeon. Make sure you are being treated by a board certified maxillofacial surgeon who has a lot of experience and training in orthognathic surgery. Best of luck!

An 8mm overjet is significant. Most often this cannot be corrected with braces alone. Combination treatment with orthodontics and surgery is the most common way to treat this issue. Discuss this with your orthodontist and a board certified maxillofacial surgeon who frequently performs orthognathic surgery before you make any decisions. Best of luck!

Ask your orthodontist if you are a class III malocclusion. Many times a protruding chin is the result of mandibular excess. If so, an osteotomy to reduce the mandible is indicated. If that’s not the case, a chin reduction would be the procedure of choice. In your case, there are several factors to consider. I do not think that it is a contraindication to perform malar augmentation in combination with lower jaw surgery. You must consider that anesthesia time is increased and as in any implant surgery, contamination is a big consideration.
