I was advised explant under local with no capsule removal or muscle repair - is this standard?
Stephen Brown MD, FRCSC
Jun 8, 2022
It's unfortunate that you've had this surgery but aren't comfortable with the way your breasts feel. I agree with the recommendation that you have been given. There is likely a very thin capsule at this time which does not need to be removed and over time, without the implant in place, should disappear. If the implants are in a submuscular position, the muscle will adopt a normal position after the implant is removed and I would not suggest repairing these fibres. Your existing breast shape...
Are my breasts tuberous? What is the best treatment?
Tuberous Breasts?
Apr 20, 2022
As you probably know, what we describe as a tuberous breast usually has a tight breast mound with a high crease under the breast and a loose and usually large areola with the breast bulging through. There is a real spectrum of breast shape from wide and round to narrow and more conical. I agree that your breast shape is more tuberous. Just using an implant will address the width problem but may not address the areolar size and bulging. This could be tightened with a mastopexy just around the...
How to make the upper eyelids less exposed and elongate the eye?
Eyelid Asymmetry
Feb 17, 2022
I agree that from the photos, it looks like you have left eyelid ptosis or droop causing an eye that is not as widely open and a high fold in the lid. This can be treated surgically by a couple of different methods with improvement but a perfectly symmetrical result is difficult. You should have an assessment by a surgeon that has some experience , either a plastic surgeon or oculoplastic surgeon. Best of luck.
Is it safe to remove implants from 1975?
Longstanding breast implants
Feb 10, 2022
If you mom has no symptoms related to her implants then I would not recommend any surgery. Having the implants left in place doesn't put her at risk. If she is bothered by pain or firmness and her general health is good enough to have an anesthetic with minimal risk, then I would recommend removal of the implants. I would suggest that she have an ultrasound or MRI to see if there is spread of any silicone beyond the implant capsule. If there is, it might make any surgery more difficult.
7 years PO, implant uncomfortable & bigger - is something wrong?
Breast discomfort and change in size or shape
Feb 9, 2022
There are many reasons for an increase in discomfort or a change in size or shape in an augmented breast. Firstly, one has to determine if this is a breast related problem or an implant related problem. This may be obvious just with examination but will almost certainly need some imaging with ultrasound, mammogram or even MRI. Insuring that the breast is healthy is the most important step.Capsular contracture could be causing your problem. How quickly it progresses is unpredictable. Some...
Procedures in 2008 & 2011 - how many surgeries is too many?
Multiple Nasal Surgeries
Jan 31, 2022
There is no question that with each surgical procedure on the nose, the surgery is more difficult to perform, the time for swelling to go away and the final result to be stable is longer and that final result is less predictable. With that in mind, if you are having problems with how your nose works because of your latest injury, it would be reasonable to treat that surgically if the surgeon feels that your breathing could be improved. The final cosmetic or aesthetic result may be more...
Too Young for Facelift and Cheek Implants?
Cheek Implants Are Okay For Young Patient, But Not Facelift
Oct 4, 2013
A facelift is designed to address sagging tissue (skin, muscle, fat), and this only occurs as we age. At 20 years old, the amount of sagging tissue would probably not be severe enough to warrant a facelift. However, this can only be confirmed with an in-person consultation with a plastic surgeon.

On the other hand, young patients can receive cheek implants if deemed medically suitable. You can also try getting fillers, a nonsurgical treatment that adds volume, to see if it is...
