Is it possible to have upper eyelid surgery with SVTs?
No Adrenaline, No Problem.

I am extremely surprised that yo have been turned down for surgery. It is actually very common for people to ask to have this surgery without the use of adrenaline. It is perfect possible to apply local anaesthesia without adrenaline. For the purposes of upper eyelid surgery, this will not impact significantly on bleeding. I would normally ask for cardiology opinion before surgery.Best of luck for your future treatment.
Do I need more skin removal after ptosis surgery four years ago?

There is asymmetry in the skin creases which creates too much tarsal show, making the lid look fat. This is possible to correct with a skin crease reformation. My own feeling is that it looks fine and the issue is only a mild one.

Marcus-Gunn is a very difficult condition to manage. The reason that botox does not work is because the synkinetic movement induces inhibition and overaction of the elevator muscle. botox here will simply cause a severe ptosis.Surgical Treatments for Marcus Gan be effective and centre around correcting underlying ptosis, and in some cases transecting the elevator and holding the lid open with a brow suspension system.

The appearance of the photos is indicative of previous filler injections, most likely a highly cohesive filler that is causing swelling. Its quite common to see patients who have had filler in the past and don’t quite appreciate the change/effect it causes, even when it for the worse.If you have had previous filler, I would get this all dissolved away and start again with a more specialised filler for the lower eyelid area that is less likely to cause swelling and discolouration.Other…

There is some significant asymmetry due to different skin creases and a small right ptosis. I would suggest a ptosis correction with skin crease reformation, and address the lower lids with tear trough/cheek filler in the first instance. You should get a very good improvement from this as a first step.

Im not sure which type of Revolax was used, but these are more cohesive fillers that can cause long-term swelling. At this point, you will need to have the fillers dissolved and a more gentle formulation injected as the tear troughs are a very sensitive area.

Im glad that you are having the appropriate investigations. You will need orbital scans and blood tests. If this it TED, you will need specialist care which may include systemic medication, and a variety of surgeries to the orbits and eyelids. Most people do well. Best of luck.
