Since your implants never sag, it is your natural skin and breast tissue that may sag with time. the size of the implants will not make this more likely. In fact probably selecting a slightly larger implant may help minimize some sagging of your breast tissue. however, for the most part, your skin and breast tissue are not affected by the implants.

either 300 or 320 cc will give you a very nice result

Please realize that the difference between 300cc and 320 cc is almost imperceptible. remember one once of fluid spread over your entire breast area will only make a very tiny difference, and you are talking about 20cc which is less than the 30cc in one once!!!
i personally would not use the low profile since you want more projection if you are doing a breast implant.

Your problem is NOT classic gynecomastia. you will not benefit from having tissue removed from your chest. you will need either a nipple reduction or reducing the tissue directly under the nipple-areolar complex. i do a lot of the latter surgeries and this can be performed in my office with local anesthetic and oral sedation (as opposed to general anesthesia in the main OR)
You will need the correct surgery to get the results you want.

Liposuction will be useless here. your problem is loose skin.
however, if you notice when you extend your arms fully above your head, then all of the loose skin will disappear. the problem is that if the surgeon removes this extra skin when. your arm is at your side, when you raise your arm above your head, you will either rip open the incision, or you will not be able to fully extend your arm. both very problematic.

How long you need to wear the arm garment should vary for each patient. How long i recommend a garment be worn depends on the skin tone. It can be from one week to 12 weeks and will vary depending on how I asses their skin tone. Not everyone is a good candidate for upper arm liposuction.

I am a huge fan of Smartlipo - also called laser lipo and i have done over 1,500 cases in my office. you don't need general anesthesia - only local and oral sedation and you can return to work the next...

Smartlipo is a fantastic way to do liposuction since it can be done in the office under local anesthetic with oral sedation. you can return to work the next day. One of the most important advantages of seeing a board certified plastic surgeon, is that he/she has other techniques available.
you almost certainly have a pronounced diastasis recti, with poor muscle/fascial tone. The only thing that will help you is a TT!!!

smartlipo may be helpful but will not make your abd perfectly flat

It is hard without examining you in person but I suspect at least part of your problem is muscle tone. Smartlipo is a great way to decrease subcutaneous fat but, it of course, cannot remove fat behind the muscle or increase the muscle tone.

you can try working hard on your core exercises like planks etc but you may have to accept a TT if you want a perfectly flat abdomen. the scars are low and do fade well. Your umbilicus does not have to look terrible either:)

I don't not believe that the non-surgical options work for gynecomastia, unless it is a very very mild case of gynecomastia and the person is only wanting a small change.
don't take shortcuts. wait until you can afford a board certified plastic surgeon with experience in this field and don't just find the least expensive dr you can find.
you did not include photos but if you have a ton of fat in your breasts, you probably are moderately overweight as well. it will not solve your problem...

I would do this case under general anesthesia and aggressively remove the glandular tissue with surgery and not liposuction. I would also place a drain for a few days - one on each side. I think if you do less than this, you will not be satisfied with the final result. See a board certified plastic surgeon with gynecomastia experience.

Do I have a high riding breast that wont drop, or do I need a revision after 6 months?

I would wait another 8 weeks but usually by 3 months they will have dropped into the correct position. If they do not drop, the surgeon will need to take you back to the OR, open the incision and lower the inframammary crease. They are not terrible, just not ideal. You and your surgeon will need to discuss the costs for this.

When you are lying flat your implants appear to be in good position. you don't appear to have very good skin tone. i always put breast implants under the muscle (for first time BA) because i feel that the muscle helps hold it in the correct position and helps limit mobility of the implants. You can get what is called hemosiderin staining after bleeding but that is much more common in an injury to the legs. I doubt very much that it relates to your previous BA done 2 years ago.

The scarring is generally well hidden since it is just along the lower half of your areola. This allows the surgeon to remove both glandular tissue and fatty tissue. I always place drains that will exit near the armpits. you can return to full exercising in 2 weeks including lifting.

The direct excision scars are on the lower half of the areola and therefore are much less noticeable than the scars from the liposuction.
