While this corrects the volume asymmetry, the breasts can sometimes have a different appearance since there is more soft tissue on one side relative to the amount of implant present. You are correct that your tattoo will be pulled closer to her waist and that the belly button will come out through the tattoo. Whether or not those changes are worth the improvement in your abdominal contour is up to you. Advancing the skin upward is described, but is not advised in your scenario. It is not as effective in tightening the skin and has less favorable scars associated with it. I would strongly recommend a standard abdominoplasty with tattoo touchup as indicated.

It is a little difficult to tell without an in person consultation, but your right breast looks borderline for needing a lift. I think that you could most likely get away with augmentation alone, but I would get a few different opinions before proceeding.

Due to the relaxation of your abdominal wall, your best result would be obtained with formal tightening of your abdominal wall via abdominoplasty. Whether a mini would be adequate is difficult to tell from your photos because it depends on the laxity of the abdominal skin. This question would be best answered in person. Liposuction alone definitely will not correct the abdominal protrusion with or without Renuvion.

It appears that she have fairly significant herniation of the nipple areolar complex. If an augmentation is performed without correction of that issue, the nipple will likely be too low on the implant and have a strange appearance. I would not recommend that course of action.

While it is possible to augment the breasts and correct most of the asymmetry with implants, the herniation of the nipple areola complex can only be fixed with an incision around the areola. I do not believe it would be an acceptable cosmetic result without correction of the second issue. I would encourage you to meet with a few plastic surgeons to satisfy yourself that the recommendations are largely the same.

Most of your aging appears to be due to volume loss rather than descent. In patients with early changes such as yours, I prefer to start with volume replacement. I prefer to take measurements, but everyone has their own way of doing things.  Just looking at your photos, I think that something in the 450cc range would be reasonable to get you where you want to be.  It's always a good idea to get a few opinions as you are doing and go with whomever you feel most comfortable.  Simulators are somewhat helpful to some in choosing a size as well.

Cup sizes vary widely among manufacturers, so that can be very difficult to pin down.  There is a maximum safe amount that can be resected without jeopardizing the blood supply to the nipple, so one must be careful about trying to go too small.  750 grams seems like a reasonable goal for you based on your photos, and maybe even a little more.  The nipple piercing should not be an issue.

Unfortunately if you are prone to thickened scars, any incision may leave an unfavorable scar.  The key to minimizing scars is minimizing the tension placed on those scars.  The incision in the fold tends to be under very little tension and usually heals very favorably.  It is also the least visible if it doesn't end up healing well.

This is a discussion best had with your plastic surgeon as everyone has their own opinions on the matter.  You definitely should not be driving if taking any narcotics or if it hurts to put your hands on the steering wheel.  There is unfortunately no real way to predict your level of post-op discomfort ahead of time.  Talk to your surgeon and make a plan togethre.
