Is getting Hyaluronidase twice in the same week okay?
better to wait
Apr 16, 2020
Dear ForeverYoung25,     I am sorry you had a bad outcome with your filler injections.   I am glad that the dissolving is helping.  I usually have my patients wait about 2 weeks between hyaluronidase injections to let all the swelling resolve as well from the dissolving.   You may find that if you wait another 10 days or so you will be much more pleased and the area will look a lot better than what it looks like now.  If you need more, I think 2 weeks would give you the best results.
What are my treatment options for puffiness/fat bags/circles under eyes from thyroid eye disease?
Graves’ disease
Mar 31, 2020
Dear npoliver,    As an Oculoplastic surgeon, this is a disease I see frequently in my practice.  For patients that have active TED (thyroid eye disease) there is a wonderful new drug recently approved by the FDA that has changed the course of this disease for many who suffer from it. It is called Tepezza and is given by IV every 3 weeks for approx 24 weeks.   It can reverse the signs and symptoms of TED and cause your swelling to recede.  I would certainly seek a consultation from an Oculo…
Is there any natural way to remove or lessen a cleft chin?
cleft chin
Mar 24, 2020
Hello, thank you for your question!  You can make a significant improvement with hyaluronic fillers like Juvéderm or Restylane in that area.   These fillers can be used to blend in the > shape that you don’t like.  In this area, there isn’t a lot of movement, and thus fillers could last you over a year.   I routinely perform this procedure for my patients.  Please seek out a qualified Oculoplastic, facial plastic, or dermatologist physician to provide this expert service for you.  Best of luck

ptosis and hollows
Mar 19, 2020
Hello, I assume your second photo is the most recent one.  Your ptosis on your left side is getting worse, I agree, and certainly may be at the point where insurance would cover it.  I would recommend you obtain a consultation with an Oculoplastic Surgeon to assess your findings, advise you, and ascertain whether or not insurance will cover it.As far as the hollows, with ptosis, because the lid is lower, you will appear more hollow.  When that is raised back to the normal position, you wil…
Can you receive under eye/tear trough filler and under eye Botox the same day?
botox and fillers
Mar 2, 2020
Dear Resourceful,    Yes, you can receive these products on the same day.  I do this routinely in my Oculofacial Plastic Surgery practice.   Please make sure you see an expert injector as you must be very conservative with both Botox and Fillers in this delicate area.
Am I a good candidate for tear trough fillers?
Fillers for lower lids
Mar 2, 2020
Good morning, while surgery will give you the best and more long lasting results, based on your photo, you should be able to get a good improvement with lower eyelid fillers.   Please make sure you see an Oculoplastic expert with significant experience in this area, as fillers in the lower lids do have inherent risks that can lead to blindness even if not done correctly or conservatively.   Good luck.

Angry brow!
Aug 14, 2019
Absolutely!  Botox, or any of the other neurotoxins (dysport, xeomin, newtox) can all be used for this purpose.  I treat patients for this ALL THE TIME.   Sometimes, you treat the patient once, maybe twice, and the muscles stop doing this permanently.  In other cases, you have to keep it up.  Sometimes, this occurs over an eyelid that is droopy, as the brain is trying to raise the lid to see better!  Or if the vision is blurry, the brow gets raised as we are straining to see better.   An ocu…
What can be done to fix my asymmetric face after Dypsort and under eye filler?
Asymmetric face
Aug 14, 2019
I am not sure exactly what your concerns are in your questions.  Can you be more specific?  I do not see any severe or significant asymmetry.  It is possible that your left cheek may be a little flatter and you may have a bit less volume on this side, which could be addressed with more filler.    You left brow is slightly higher than your right side and you have a bit more show of your eyelid.  You could drop this brow a little more more dysport, or raise the right one.   I think an in per…
Do I need ptosis surgery or upper eye filler?
Eyelid concerns
Aug 11, 2019
Dear Karl, it is impossible to answer your question with this single photo.  You are smiling in the photo which completely changes your eyelid position.  Smiling makes the eyes smaller, so hard to judge the lid position.  Ptosis surgery is done when the eyelid edge is too low and covers part of the eye.  Fillers are put in the eyelid if there is too much hollowness beneath the brow.  Totally separate problems.  You may need, both, or neither!  Please see an Oculoplastic surgeon for an person…
What is the best way to treat the crepey skin under my eyes?
Lower lid options
Aug 2, 2019
The lower lid is notoriously a challenging area.  There are so many factors involved, such as fat herniation from behind your eye to give you bags, skin texture changes, thinning of the skin so that darkness can be seen,  loss of collagen/elastin, and loss of midface volume adding to the hollowness and contour irregularities.  An in-person consultation would be best to assess which of these problems are contributing to the appearance you are unhappy with.   I do a combination of surgery, fil…
What causes entropian after upper/lower eyelid surgery and what procedure can correct it?
Entropion
Aug 1, 2019
I’m not sure if your entropion is caused from upper or lower eyelid surgery, As you have not noted which eyelid is effected.   There are several causes of entropion, most likely there was an inherent weakness of the lower lids to begin with that will cause the entropion of the lower lids.   Some people have an autoimmune process called Ocular cicatricial pemphigoid that can also cause scarring and entropion of both the upper and lower eyelids after any Ocular procedure. Certainly I would…
Bells Palsy and Botox/Fillers… is it safe?
Yes, but very carefully
Aug 1, 2019
Dear Gapeach, First of all, I am very sorry that you ate going through this. Bell’s palsy can be very scary and frustrating. The fact that you are 95% recovered within 30 days is amazing amazing progress.  depending on where you want your Botox and filler‘s I think it could be performed at this point but very judiciously. I would 100% see an oculoplastic surgeon who is very comfortable treating and managing Bell’s palsy. Inherently, there will be more weakness to the muscles. This must to b…
Can I do Botox and then a Photofacial (IPL), a week apart?
Ipl and Botox
Jun 22, 2019
Yes, that should be no problem to have Botox and photofacials/IPL a week apart.  The only thing I tell my patients to do is to avoid exercise and lying flat for about 2 hours after Botox.  Obviously with the photofacial you will want to avoid sun exposure before and after and any medications that may be photosensitizers as well!  Good luck!
Alternative to fillers? I degrade them too fast. I can’t keep spending over $10K a year to not loo
Other options besides fillers
Jun 22, 2019
I would definitely consider fat transfer, but definitely seek out an expert facial plastic surgeon with lots of experience.  Another option would be to consider more permanent filler solutions such as Bellafill.  You also may be a candidate for sculptra, which works by helping you create your own collagen and can give you natural, long term, volume restoration.
Can dermal fillers or Botox fix upper eyelid retraction caused by TED?
asymmetry of lids due to TED
Jun 18, 2019
There are several options to address your concerns without surgery.  Botox can be used in your upper eyelid to bring it down a little.  In the picture you posted, it appears that your left upper eyelid is higher than your right one.  However, you will have to try different doses to get the right one, and in fact, if too much is placed, your left upper eyelid may be too low as well.  So it will perhaps take a few treatments to find the right dose, and of course will need to be repeated eve…
