Can you advise on pollybeak deformity from my rhinoplasty over a year ago?
Tip Issues
Jun 23, 2021
Thanks for the message and photos. I agree with other answers in that you don’t seem to have a polly beak. However, you have a poorly supported tip that has resulted in alar retraction, a retracted tip (probably contributing to the look of an overly prominent bridge) and an over-rotated nose with very visible nostrils. You will probably need a revision to fix many of these issues with addition of tip support. We could definitely give you a refined looking nose with a better tip/dorsal…
Am I botched? Nose still has small hump, still big 18 months after Rhinoplasty. Do I need a revision? (
You will likely need a revision
May 15, 2021
I am so sorry to deliver this news, but I don’t think there is any way to provide the result you want without another surgery. While I agree that the bridge probably needs to be lowered a small amount, I think the biggest issue is the lack of support to the bottom portion of your nose. Your poorly supported tip has probably dropped down and the middle portion of your bridge has weakened. This gives you the appearance of a dip in the middle of your nose, further accentuating the bridge. …

Neck Swelling After Facelift
Dec 29, 2020
Thank you for the question and photos. Your neck and jowls look amazing!! It is nice to show the before photo. Areas of swelling and irregularity in the neck after a procedure like this is common and should go away with time. The photo angles make it a little difficult to compare, but it looks a bit better even from the second picture (probably about a week or 10 days out) to the third and fourth picture. Keep being patient, I think it will get there.
Would liposuction be effective for my double chin?
Liposuction below the chin
Nov 11, 2020
You seem to have a little pouch under your chin that looks like it would respond well to liposuction. While I am generally not much of a fan of Kybella, a very small collection of fat like this, Kybella might also be an option. During an in-person exam, we could pinch and we would be able to tell if the fullness is subcutaneous fat (just below the skin and easily liposuctioned out) or below the platysma. While you can still remove fat deep to the platysma, it is more difficult and carries…
Should I follow what the oral surgeon recommended?
Imaging for a Chin Implant
Dec 13, 2019
Chin implant is a wonderful operation with a quick and relatively painless recovery and I think it superior to a genioplasty.  During a consultation, we can use our 3D Vectra imaging system to simulate the size of the implant you would want.  We take the image into the OR and match the image using different sizers before placing the actual implant that will stay forever.  Having the actual image, predetermined together what we want, is very important to make sure you get the size and sh…
Need a chin filler estimate: How many syringes of filler would it take to achieve this result?
Filler for the Chin
Jul 8, 2019
Hi Kalie.  Thank you for the message.  I would guess that you would need 3 syringes.  Either Voluma or Radiesse works nicely depending on exactly where you are wanting the product to go.  To be perfectly honest, I think a chin implant would work as well or better than filler.  Plus, the results would be permanent (although easily reversed if ever needed).  We use solid silicone implants made by Implantech.  They work really, really well with sky high patient satisfaction.  I hope that in…
Do I need an alarplasty revision?
Revision Rhinoplasty with or without Alarplasty
Jul 1, 2019
Thank you Skyyylar for your message and for the photos.  I can definitely see how markedly upturned your nose looks but it is a little difficult to fully assess the alar base and nostril shape from the single view I could see.  But, pushing the tip down should not only make the nostrils less visible (you don’t really want to stare into nostrils after rhinoplasty) but will likely alter the shape and make them have a more natural appearance.  Alar rim grafts are also helpful as changing the sh…
Can I do a lower blepharoplasty with fat repositioning in office?
Lower Bleph Under Local Anesthesia
Mar 27, 2019
So, I guess technically anything can be done under local anesthesia (there are some brain surgeries done under local so you can respond when they stimulate a specific area).  While I believe there may be some willing and patients who could tolerate it, I haven’t ever.  This is a very delicate operation with tiny changes making the difference between a good vs a mediocre outcome.  So, I worry that the patient might become uncomfortable, squint, move, or do something that may make us rush more…

Tip De-Rotation
Mar 27, 2019
Thank you for sending excellent photos.  This really helps when trying to offer an opinion on others work.  Obviously, you are very early on in the recovery process.  So, it is difficult to make complete decisions.  However, there are several areas of significant concern.  First, the over-rotated appearance.  While it is true that the tip may drop a little over time as your nose heals, it is very over-rotated and it will never make it to 90 degrees (generally I prefer a slight up turn of 95…
Nose job and chin implant for better profile – Will I be able to achieve my goal from these pictures? (
Rhinoplasty and Chin Implant
Mar 20, 2019
From what I can see in your pictures, I think your plan is fantastic and completely agree.  You have some features to your nose that are nicely corrected with rhinoplasty (slightly elevated bridge, tip that points a little downward, wide and rounded tip, etc…).  You also have some weakness to your chin that would look amazing with a chin implant.  I think these procedures would help provide a little better balance to your face, similar to Selena.  I hope you visit our website for many exa…

Revision Rhinoplasty
Feb 15, 2019
Thank you for the question and for the photos.  From what I can see, I don’t think that loosening a suture will provide the solution.  There seem to be several issues including an elevated bridge and/or middle third saddling, over-rotation, nostril flare, and the angular straight columella is the result of a lack of an infratip break.  I don’t really see how you will get the result you want without a full revision.  As has been said, I would wait until things are fully healed (usually aro…

What could my nose look like after rhinoplasty?
Rhinoplasty Imaging
Feb 11, 2019
During a consultation, my goal is to show you my view or vision of what I would want to achieve.  Really, during the consult, we are painting a picture and I want to make sure that what I am putting up on the canvas is exactly what you want.  I feel this is best done with photo imaging (done with 3D Vectra imaging program) and using before-and-after pictures from our gallery.  If you go to our website, you can see almost 150 examples (more than anyone I have found in the intermountain we…
Recurring infection post rhinoplasty. What could it be?
Chronic Infection After Rhinoplasty
Feb 11, 2019
I have seen this once before.  A patient had a rhinoplasty where a braided permanent suture was used.  We had to widely open her nose, remove every single suture, flush it with antibiotic solution and then close.  No new infections and her nose looks awesome.  Do you know what kind of suture was used.  A culture of any of the abscess fluid would also help but if there is still foreign material (which I suspect is the case) in your nose, you have to get 100% of it out. The infection will n…
Follow-up question: Should I get a revision rhinoplasty?
Revision Rhinoplasty
Jan 11, 2019
Thank you for the email and the photos. I hate to be contradictory, but I don’t agree with your surgeon’s assessment or recommendation. I think there are some significant unresolved or new problems, particularly with the tip and I don’t think a steroid injection will fix it. You have poor tip definition, a hanging columella, tip asymmetries and a supratip that is still just a little bit too high.  All of this should be nicely corrected with a revision surgery.  If you go to the link be…
