There re several less invasive procedures that are available. Ulthera is often effective for early facial sagging, and can give a nice result. It requires an extremely skilled operator to give a nice result without a great deal of pain, and these can be hard to find. It is the only procedure that is FDA approved to "lift" the face. Sorry to hear about your problem. Stretch back of the skin and muscle can occur if the skin is not young and elastic. Any type of facelift that you have should have a mid-face included. I prefer deep plane facelifts with the mid-face. This relieves the tired, mad, sad look that we all get as we get older. The mid-face is an essential to relief the RBF. Only about 5% of surgeons perform the mid-face. Look for a board certified Facial Plastic Surgeon to ask.

I have used this technique for over 30 years, so feel confident, from your pictures, that you would get similar results to what you are showing. Of course, a consultation is required. This technique can be performed in several ways, but is tricky to do. In my experience, only about 5% of surgeons are confident with it. It is minimally invasive, and, in many people can be performed with local anesthesia. I've taught to many of my UCLA facial plastic surgery fellows.

A coronal lift severs all of the nerves that go to the forehead, so numbness last longer. It does permit work on the bony structures of the forehead and brow. If the hairline is high it can raise it.

An endoscopic brow lift is less invasive and offers similar results. My feeling is that, under magnification, the muscles can be tailored better. It usually does not lift the hairline. It has advantages over the trichophytic lift, as it only uses vertical incisions within the hairline...

Sometimes, widely spaced muscle bands can be related to digastric muscles. Assessment of this problem and the quality of the neck skin is difficult with the pictures that you have.

A cheek lift (which can be performed under local anesthesia) is probably your best option (though an in person or remote consultation is important to be sure). We have performed these for at least 30 years, and the results last 7-15 years. Long lasting fillers have complications (granulomas, sculptomas, lumps). HA fillers are a good option, but need to be repeated in 6 months to a year.

The platysma muscle can be stubborn, but a re-operation will usually correct the problem. Sometimes the band is not the platysma, but can be a digastric muscle band, particularly if there is more than one or previously, the neck was opened, or if there is a "lump" under the neck. A revision procedure usually just utilizes a scar under the neck, and can often be done under local anesthesia.

I cannot resist answering this question. The procedure is tricky to do, but with a surgeon who is experienced with the procedure and a patient who is a good candidate, will result in a restoration of the cheek position. A good test to see if you are a candidate is to smile. If this provides an elevation of the cheek, it is likely that you are a good candidate for the procedure (the procedure will usually reduce the depth of the nasolabial fold, not increase it, as the smiling does).
