Congratulations on your weight loss. You are fortunate in that you have a narrow waistline that will show well with removal of your excess skin. An abdominoplasty will correct skin excess on the front of your abdomen, but a lower body lift that extends 360 d. would enable us to improve your sides and backside. Be sure to see a surgeon who has experience with belt lipectomy or lower body lift.

If you now have liposuction, you are likely to be left with loose skin again, and since you already have brachioplasty scars, there is no question that your best option to tighten your skin after the liposuction is to have further skin removed (brachioplasty). Energy based devices can produce some tightening, but it is likely to be irregular and vastly less effective than a brachioplasty.

The relationship of your nasal base and cheeks creates the upper nasolabial fold, and fillers would be the best option for improvement in a 22 year-old. It is good that you are aware of the risk of intra-arterial injection, but in skilled hands the risk is extremely small and there are many things you will do today that carry greater risk. Good luck!

Provided the implants are narrow-based, and pocket dissection is cautious, breast augmentation should minimize the appearance of symmastia. Selective liposuction may also be a useful tool, particularly if you did not want larger breasts.
