Definitely only work with surgeons who specialize in your anatomy and condition. The surgery would consist of advancing the levator tendon some more (perhaps for both sides) and using an anchoring technique to keep the crease in place. Maybe? Probably. We can't tell really without a photo but more importantly and good exam. HOWEVER, a medial epicantoplasty performed with a traditional "V" or "W"-plasty always shows a scar. Always, even if it's not that bad, you can see a little something.That being said, thicker skin often times leads to more visible scaring. But so does very elastic skin or very good skin. Let me give you a little advice. One, yes, it's asymmetric and you may have had a bit of Herring's law happen after your ptosis surgery. So yes, maybe you could stand to have ptosis correction on the previously uncorrected eye. Two, if the high or "big" eye is the one that had a ptosis surgery then it wen well. That's a tough one. Believe it or not it is often possible to have an Asian Blepharoplasty aka Asian Eyelid Surgery without adding a crease. If you want a small crease, that may be possible. If you could provide some photos that would help.

With regards to the top bit of the ear we (I say we on purpose) got older and the tissue (all of it) is less resilient. Not the bit on the bottom in the back. . .possibly developed a very late neuroma. I'd suggest some kenalog shots or PRP shots first. That's probably help. You simply cut it out. You then suture it closed. That's about it. There is no such thing as no scar but ears do tend to scar a little unpredictably. So be careful. If you're really bothered by an extra piercing hole then YOU'D REALLY BE BOTHERED but a noticeable scar. Leave it alone if you're already stressing at the thought it not going well. I will say as you get older it may make more sense to remove the holes and your odds of a bad scar lessens.Chase Lay MD
