I would suggest first getting a good history and physical, complete with blood work. This will rule out any thyroid or hormone issue. Once that is taken care of, yes BOTOX and fillers can be a great tool to help brighten up a face. Especially if you are running long distances, that could account for the facial volume loss.

Steroids are a common treatment for skin cysts. They can shrink a cyst and keep the patient from needing an excision of the cyst which will result in a scar. Is the hollowness from the steroid or just lost volume from the cyst. Hard to tell. Most doctors that inject the cysts use a very dilute steroid which generally has little effect on the surrounding tissues. You have a couple issues contributing to the dark areas under the eyes. First, I would suggest sunscreen and a topical hydroquinone to work on the skin. After a few months if you are not seeing the desired improvement, you may want to consider a chemical peel. You also have some hollowness undr the eye. Yes we do have some fillers that can last up to 18-24 months for most patients. It is always an experiment to see how long they last in your body-could be longer or shorter.

Your goals are not seeing an ethnic rhinoplasty but more of a caucasion nose. The skin of your nose appears very thick compared to Claudia's. Congratulations. At the end of the day, all of our bodies will hold onto the fillers differently. It sounds like yours really likes it. Our bodies will lay down a small amount of collagen, but generally for most patients that is a small amount. I have seen patients hold onto it for 5-6 years. If you develop asymmetries they can be dissolved.

You are lucky you work in a dental practice. You are correct that to achieve the best positioning of the lips and profile, it would require orthognathic surgery. A chin implant is a nice procedure to advance the chin forward but the over jet will still be present. If you want to see the aesthetic impact of advancing the chin forward, you may want to try a filler such as Juvederm Voluma. If you are pleased with that look, an implant may work for you.
