Great question and a common one. You do have a rounder, more bulbous tip, which isn’t harmonious with the rest of your oval shaped face. I agree, a bit more definition to the tip will be advantageous. As we age, this only gets worse as the ligaments that hold the cartilages close together become more lax. Reshaping the tip with cephalic trims, lateral crural strut grafts can be done and tip stability can be accomplished with a caudal septal extension graft. I’d favor lifting your tip ever so…

Great question, typically for an Asian rhinoplasty, we aim to build the bridge and raise the radix (start of the nose). To do this, all of those techniques are employed. Our preference is either using ear cartilage or rib. There is a lot of experience in Asia with silicone and goretex. The downside of the synthetic materials is the risk of infection and extrusion, goretex is notoriously hard to get out in that circumstance. Nice job with the photo editing! I think you are a good candidate, with much of the work being done to refine your tip. You have what we refer to as a bulbous tip, the goal would be to reorient the lower cartilages so that they lie flatter. This can be done with lateral crural strut grafts, possibly alar rim contour grafts. A small shave of the hump and you’re all set!

Indeed you did have a beautiful cosmetic result, congratulations to you and your surgeon. The mass in question is hard to identify without full examination, but if I had to take a guess, it would be a turbinate. It is exceptionally rare for a polyp to develop in such a short amount of time and I would suppose that if it existed at the time of surgery, your surgeon would have removed it or taken note of it.

Appreciate the drawing efforts, but we can only judge appropriately with an actual photo. In general, this area has a robust blood supply and will heal well. Make sure to keep the area moist with an ointment and follow up with your surgeon at the predetermined time.

Hard to say without examining you or without standardized photographs, but you look like as you should just 10 weeks after rhinoplasty. This is the time that you will start to see definition (as we do in your case), but certainly you have a way to go. About 30-35% of the swelling still remains, much of this in your tip. We do see this, depending on what type of anesthesia and what was done during your surgery. Here are some possibilities:
1. General anesthesia with orotracheal intubation: manipulation of your jaw to get the endotracheal tube in may have caused swelling.
2. Surgery: any surgery near this area, say for harvesting of a temporalis fascia graft (do you have an incision in your hairline) can cause swelling of the muscle that controls the jaw opening.
