I’m sorry that your outcome was not quite what you were expecting – this is a common issue in women with large areolas, but it’s of course difficult to deal with when faced with the problem in the preop area. Based on that photo, you do not – canthal tilt is simply looking at whether the lateral (outside) canthus (corner of the eye) is positioned higher or lower vs the medial (inside) canthus. For a negative tilt, it would be positioned lower, creating an unattractive “sad clown” appearance.
