The lateral bulges either represent dehiscence of a platysmaplasty suture with relaxation in the midline or a deep plane technique done without any platysmaplasty. The large bulges on each side of the neck are likely to be salivary glands. Jowls are defined as bulges along the jawline. Your jawline is smooth, and you have no jowls. The bulge in the midline is likely a suture knot if a platysmaplasty was done. The horizontal band may be due to a horizontal pull of the tissues impeded by the large salivary glands. It looks like either the platysmal sutures were dehiscent or possible resection of midline fat under the platysma or prominent anterior digastric muscles. This cannot be corrected by more of the same technique. A routine platysmaplasty with a SMAS lift may not correct your problem. For this, the area under the platysma in the midline and from the lateral approach is done with my technique. This is performed in the same plane as a deep plane lift.

Whenever fat reduction is done in the form of liposuction, it is possible that muscle structures under the skin become more visible. The vertical bands are platysmal bands, and the round structure is likely a salivary gland. I don't see a submental incision so I'm not sure a platysmaplasty is done. This probably will require revision surgery. If you have this redone, it needs to be a different technique to address the bands and glands.

The deep plane lift is a very extensive facelift. A refresher facelift sounds less extensive and might even be a mini-lift. You definitely need a face and neck lift, A deep plane would be my preference but a well-performed SMAS lift with a platysmaplasty would also give good results, depending on the surgeon.

The problem is not with persistent loose skin. After two surgeries, your submental neck has become skeletonized due to the loss of a fat layer to hide irregularities. Usually I do not recommend non surgical treatments such as Morpheus or Facetite, but in your case I would have you consider it. You have a positional area of looseness. Usually this positional problem cannot be corrected with facelift surgery because all lifts are judged in the facial rest position and not with your chin to your chest. Most facelifts show improvement of the marionette lines but not the nasolabial folds. Another set of problems has developed with your approach. Your platysmal bands have been exposed now that your fat has been removed. A mini-lift does not address this problem because it is too close to the middle. Mini lifts do not address the submental neck well. At this point, you can try botox to soften the muscle bands. A neck lift with platysmaplasty is the next step if that does not work.

Your painful pea-sized bump might represent a possible suture granuloma if one was placed in that location. Treatment might require exploration and removal and/or antibiotics.
