If you do not want to have fillers & neurotoxin, lower eyelid fat transposition blepharoplasty can help with the contours and “tired” appearance. Surgery won’t, however, help with the lines around the eye area. Perhaps a consultation with an oculoplastic surgeon who is well-versed in the use of nonsurgical as well as surgical modalities can help assuage some of your concerns regarding fillers & neurotoxins?

Hello. Cheek implants will help to augment the cheek and hide the lower eyelid “bags.” I often do lower eyelid fat transposition blepharoplasty in conjunction with cheek implants to achieve maximum improvement, but it’s fine to do it in stages if that’s what you and your surgeon are planning. Fat/filler injections will only give you a mild improvement.

Hello. The photo shows hollowing and dark circles under the eyes, but I can’t see the veins you’re referring to. Typically, lower eyelid veins are located at the outer corners and can be treated with laser, surgery, or injections to collapse the veins.

I agree with some of the other doctors that an examination is needed to see whether the filler contributes to the contour irregularities. But since you also have more “crows feet” lines on the R side when you smile, I do think that at least a part of the issue is due to dynamic muscle movements. Hello. While I agree that you need to have ptosis surgery on the left upper eyelid, I don’t think you should have further blepharoplasty surgery. The tarsal platform show on the left side is already elongated and ptosis surgery should make it symmetric with the right side. Hello. You’re now in the cicatricial (scarring) phase of healing right now, when things look worse before they look better. From now through postop month 3, steroid+5FU injections, massage, and UV protection will help. Revision surgery, if needed, may be undertaken as soon as postop month 6 but the longer you wait (until 9-12 months if possible), the better chance of success with revision surgery.

If eyelid skin was not excised during the initial surgery, the eyelid position may still improve with time. Based on your photos and how long it’s been since surgery, however, I suspect you will need revision surgery. Please visit your surgeon to see if scar modulation measures can be immediately pursued, and whether plans for revision surgery need to be made.

Without seeing your photos, it’s hard to tell whether you have pseudo-strabismus (the appearance of eye motility/alignment problems usually due to the eyelid anatomy).
