Hi jamburst1. My analysis of your photos primarily shoes an overly upturned (or "rotated) tip resulting in a short nose with excess nostril show. You would likely benefit most from counter-rotating the nose and using asymmetric grafts to help lower the nostrils more on the left than the right. Because we know you are missing cartilage from your nasal septum, it is distinctly possible you will need more cartilage from another area (ear/rib) to make significant changes to your external nasal shape. Sorry for the misleading title, but this reminds me of when a patient asks if we can use a laser instead of some other tool. Sometimes that is a good idea, sometimes it is not. Similarly, piezo is a tool to make cuts in the bone during rhinopolasty. Traditional techniques using sharp osteotomes to make the cuts are another technique. Most patients do not require alar base reduction with mild tip deprojection.  It is difficult to assess if this would be necessary in you without an frontal, and ideally, a base view.  That said, there are option including a base-cinching suture that may achieve the same goal without base reduction incisions.  If they are necessary, dermabrasion afterwards in the office may eliminate any noticeable scarring.  Best of luck.

The junction between your nasal bones and cartilage is wide, creating the bumps you see on the sides.  This is correctable with rhinoplasty, as are the other concerns you express.  Visit a rhinoplasty specialist (Facial Plastic or Plastic Surgeon) that will perform computer imaging to demonstrate what changes are reasonable to expect.  Best of luck!

I agree that reduction of your small dorsal hump will improve the appearance of your tip projection, but this could be further enhanced with tip cartilage grafts.  The preference of endonasal (i.e., closed), open or non-surgical rhinoplasty is really surgeon-dependent.  That said the non-surgical options are riskier in the tip area.  Your skin thickness is a little more challenging but should be manageable by an experienced rhinoplasty surgeon.  I would make a consultation with a Boa...

While most patients do have some degree of facial asymmetry, this can often be improved upon (not corrected).  In your case, you have inferior scleral show of both eyes (white showing beneath the colored part), indicating lower lid retraction.  Merely repositioning the corner of the eye (canthopexy) would help, but your best bet would be to have grafting inside the lid in addition to the canthopexy.  I would  recommend seeing a Facial Plastic Surgeon or Oculoplastic Surgeon specia...

Oyatanya,I tell my patients they are all going to hate their nose at first,
second guess their decision, etc. (they don't always, just want to make

Eyelid surgery and rhinoplasty are commonly performed together.  Briefly, although limited by the lack of a physical exam, it appears you have some loss of volume to the upper cheek/tear trough as well as some "eye bags" caused by fat pushing out from around the eye.  There are many options to treat this although with varying results.  My recommendation would be either volume alone with fat transfer/or fillers versus a combination of volume with lower eyelid surgery.  The latter, alt...

Unfortunately, no enhancing your bridge is not likely to change the appearance of your epicanthal folds.  However, providing more shadowing along the sides of the bridge may help the overall appearance as it will allow for a less "flattened" look.  I would make sure you consult with a surgeon comfortable and experienced with rib cartilage grafting so you can discuss all options available to you.
