It appears that your left upper eyelid (right side on the photo) is slightly lower and you are compensating by lifting your eyebrow. You likely need a revision of the ptosis repair on that side. Ptosis repairs have a high rate of revision due to many factors including neural input from the brain and due to the the muscle moving during the healing period. I recommend you seek the opinion of an oculoplastic surgeon that does a lot of ptosis repairs.

The "sleepy" appearance of your eyelids is due to ptosis, or drooping of the eyelids. This can be repaired by a surgical procedure that tightens the lifting muscles of the eyelids. This is a routine operation and can be performed under local anesthesia. I would recommend seeking the opinion of an Oculoplastic Surgeon. In my office we can demonstrate what the result would be by placing eyedrops that temporarily lifts the muscle.'

Your photographs show a combination of hyperpigmentation and lack of volume in your tear trough. I am not sure why an MD would recommend weight gain as an answer as gaining body weight won't fill in the tear trough.I think you would benefit from tear trough filler and skin lightening creams. I recommend you seek the opinion of an oculofacial or facial plastic surgeon that treats this area on a routine basis.

The first photograph that you posted shows some tear trough hollowing and fatty prolapse. I do think that you are a good candidate for a transconjunctival blepharoplasty with either fat repositioning or fat grafting. You also seem to be a good candidate for tear trough filler. Tear trough filler may allow you to put off surgery for several years. It is hard to give more advice without seeing you in person. You should speak to your surgeon about your concerns.

It is difficult to tell excactly what is happening from your pictures.  The junction between the cheek and lower eyelid (tear trough area) is a common place to have loss of volume which can result in dark circles and an aged appearance. If cheek filler is administered without addressing the tear trough the appearance of volume loss can be worsened. You should speak to your Surgeon or seek the opinion of a Surgeon skilled in injecting tear troughs.

Radiesse is an excellent choice as a filler for the Jaw line. It has a very high G-Prime (stiffness) and it can help create a more defined jaw line and chin. In general the treatment lasts 12-18 months. It does have some mild collagen stimulating effect so with repeat treatments there can be some permanent change. Sculptra is a better collagen stimulating injectable treatment but it takes several treatments to obtain the desired result.

Your photographs don't show much hollowing and in fact you have very youthful looking lower eyelids. For minimal hollowing I recommend lower eyelid fillers. This should always be performed by an expert injector. My preferred method is to use a cannula to reduce bruising and to improve safety. HA type fillers can last a long time in this area and can be safely and easily dissolved if you are unhappy with the results.
