I would have to exam you to determine your options. In general, If pre-planned using a 3D CT and cutting guide, you can obtain the desired results within biological limits. A custom chin implant may also be an option.

When I left residency from Vanderbilt 15 years ago, I felt that genioplasty was the only surgery for chin advancement. At the time, chin implants were very limited. Things have changed. chin implant can give a very natural look and are less traumatic to place than doing a genioplasty. I had to make the same decision for myself. I elected to do a medium extended chin implant. I have been extremely happy with the appearance and feel of the implant.

The way that I determine how far a mandible comes forward in orthognathic surgery or how far a chin comes forward or down is with a cephalometric x-ray. By looking at the measurements of this x-ray you can determine how far the chin needs to come forward and then pick the implant size. At the time of surgery I try in a sizer implant of that size and the next size up. Based on this evaluation and the patient's desires I will make my final decision.
