If you are asking how to rid yourself  of melasma forever, well, it won't happen.  This condition can be treated, rather successfully, with lasers (we use the state of the art Enlighten Pico laser) and various lightening skin agents.  But it will come back, with time, and that amount of time is highly individualized.

No, what you describe should in no way affect the outcome of your PRP therapy.  Please realize multiple procedures are usually necessary, and results take at least 6 months to appreciate, maybe even a year.

Your photos show you do not need much improvement, so surgery would be an extreme procedure, but threads would be a great choice.  Choose your provider carefully, that is the main thing, to look for.  Threads work immediately, last for years, and you are an excellent candidate!

You can not significantly improve your penile length, especially erect length-so I would advise you to not even really look much into that.   
You CAN increase girth or circumference.  This can be through a series of injections using dermal fillers or fat transfers.  Most patients choose the dermal fillers, but you would have to speak to the provider on this.  Girth can be increased up to an inch or more in some cases, some even reporting increases up to 2 inches.

At your stage in life, you should have your hormones evaluated first.  An O shot won't hurt, but you most likely would benefit from hormone balancing as a start.  Of course an O shot could be done as well, but I would not tell you, without knowing more, that it would be the end of the story.   Of course find a skilled and experienced provider!
