Both above and below the muscle can be made to look very natural or very not-natural to fit your goals, depending on the size/profile of implant, implant cohesivity, and your breast and chest wall shape. Point taken about not wanting your breasts to overshadow your torso given that you are petite. I recommend patients go through a sizing process where they try on sizers of various volumes to find the size they like on their bodies. That coupled with breast measurements and going over goal photographs really helps find the right size that fits your body and the look you’re seeking.
Do I have to dissolve filler in my nose before rhinoplasty?

There should be an attempt to dissolve the filler in advance, which may take one or more visits depending on how much filler you have in there. I like to do the last dissolving at least two weeks before surgery. It’s up to your surgeon, but to me it’s OK if some of it can’t be dissolved–this is not uncommon and sometimes we can cut out remnant filler during surgery, or sometimes some will be left behind. Yes, changes to the shape of the nose are to be expected after double jaw surgery (or specifically any surgery where the upper jaw is moved), so it’s very common to plan for or desire a rhinoplasty after double jaw surgery to bring you back closer to your original or desired nose shape.

Yes, prior energy treatments and sculptra do make a facelift somewhat more challenging to perform. That being said, it is certainly not impossible, and anyone who regularly performs facelifts in this day and age is very used to doing facelifts on people who have had prior tightening treatmnts, fillers, threads, etc.

Yes, this irregularity is likely related both to your initial aging and to how your facelift was performed. Revision facelift would release and redrape this area, and I would couple it with fat transfer to the oral commissures at the same time to help smooth the area and improve the skin quality and texture.

First off, you don’t know that 245cc will give you the look your friend has–she started with a different amount of breast tissue and a different chest wall than you to begin with (unless she is your identical twin!). That being said, you shouldn’t go into a breast augmentation not feeling confident about the sizing–you can decrease the size of the implant while still using an implant that is wide enough by choosing an implant with a lower profile.
