RS: What sets your practice apart?
EJ: Expertise. I’m highly specialized in all types of rhinoplasty procedures—primary, teen, revision, functional, and ethnic rhinoplasty surgery, as well as permanent nonsurgical rhinoplasty.

With rhinoplasty surgery, your surgeon’s experience is critical for achieving a favorable result. Rhinoplasty may be one of the most challenging plastic surgeries, and it’s best to get it right the first time.

Another point of differentiation is my expertise with the off-label use of the permanent filler Silikon 1000. It’s the only filler I use. Anything you can do with regular filler, you can do safely and effectively with Silikon 1000.

RS: What inspired you to become a surgeon?
EJ: Both of my parents are doctors, so it’s in my DNA. But the day I walked into the operating room, I was mesmerized. I knew part of my work as a physician was going to be in the OR.

RS: How did you decide to specialize in rhinoplasty?
EJ: I did a fellowship in facial plastic and reconstructive surgery with a world-renowned rhinoplasty specialist. We did over 150 rhinoplasties together, and I caught the bug. I love rhinoplasty, and I’ve figured out a technique that works. As a result, I have more than the average surgeon’s number of positive reviews.
I also have one of the largest before-and-after galleries of rhinoplasty surgery on the Northeast corridor. I pride myself on posting every view of every patient—not just the best one.

RS: What’s your practice like?
EJ: We’re a four-person shop, and our goal is to treat you like a member of our family. When patients come to my office, it’s a very intimate experience. The first and only medical professional they see is me.

RS: How would you describe your approach to plastic surgery?
EJ: Cautious. I’m a conservative surgeon who favors a nonsurgical approach whenever possible, reserving surgery as a last resort. For each of the 100-plus surgical rhinoplasties I perform in a year, I do three times as many nonsurgical rhinoplasties.

In my opinion, the main reason to stop smoking before surgery is because tobacco is a lung irritant. All of our patients who undergo rhinoplasty surgery received general anesthetic with inhaled anesthesia. As long as you do not have chronic bronchitis with daily coughing from chronic tobacco use, in all likelihood, surgery will be fine moving forward from an anesthesia standpoint. I hope this helps! Based on your photos, you appear to have a crooked nasal appearance and a deviated nasal septum. Generally, problems like this are repaired with septal rhinoplasty surgery. Based on your singular photo, you appear to have very thin nasal skin and firm nasal tip cartilage which is Associated with visible irregularities after rhinoplasty surgery. The soft area just next to the ridge is called the soft tissue triangle. Depending on your examination, a few droplets of Silikon-1000 maybe considered for softening this transition permanently. Empty nose syndrome is associated with excessive removal of turbinate tissue.

Empty nose syndrome is not associated with septoplasty nor adenoidectomy. It is an extremely uncommon situation, and we have not seen one case in 24 years. I am not aware of empty nose syndrome being associated with turbinate reduction using either radio frequency or electrocautery. I believe it's only associated with excessive resection. In that regard, if your surgeon is planning a turbinate reduction, this should not be problematic for you in all likelihood. I hope this helps!

Is there a reason why one cannot only put 1mls syringe of saline into 100 units of botox?
Reconstitution of Botox with 1 cc would lead to waste.

Every time a syringe is filled with botox, there is dead space in between the plunger of the syringe and the tip of the needle. If the reconstitution of 100 units is only with one cc, there would be many units lost in the dead space of the syringe. Additionally, I feel there would be a less accurate and precise treatment. Generally, the projection of the nasal tip should be equal to or slightly less than the length of your upper lip. You appear to have a pleasing nasal tip appearance on front and profile view. You may have somewhat of a deep Radix. If the top of your bridge is a bit too low, the part in between your eyes, it can give the illusion that the lower third of your nose is over projected. If this is the case, adding volume to your Radix may be achieved with either a temporary or permanent injectable...
