You have excess skin in both upper eyelids but more on the left. A difference in the amount of skin in the eyelids is actually common. Upper blepharoplasty would be done by removing more skin on the left side than the right in an effort to minimize the hooding/drooping and balance the eyelids. You also appear to have fullness in the inner corner of the upper eyelid, particularly on the left side. This is typically due to an overly prominent fat pad that can be reduced.

Based on your frontal photograph with your face relaxed, I would say that tip refinement is not imperative and that you should be able to obtain a very nice result only addressing the dorsum. In my opinion, if you are happy with your tip (or any body part), you should not change it unless there is a clear imbalance with the rest of the nose.

A purely transconjunctival blepharoplasty only addresses the fat that causes "puffiness." Treatment of excess skin can be managed by excision (resulting in a well-hidden scar under the lashes) or non-surgical means such as laser or Plasma Pen. It appears that you may be a candidate for non-surgical management of the skin. If you have loose/excess skin in your lower eyelids after this, touch-up with skin excision can be done under local anesthesia.

Lower blepharoplasty is an acceptable option for you. Based on the photo, I would recommend conservative transconjunctival fat removal or repositioning to reduce the tear troughs. You do not appear to need skin removal. Fat grafting in the eyelids can be risky because of visible lumps, which are very difficult to correct.

I feel that your aesthetic goals for rhinoplasty can be realistically met. Your nose can be made to look smaller by reducing the bump on the bridge of your nose and refining your tip. Thick skin can diminish the degree of visible change in the tip that is achieved at the cartilage level. However, I would anticipate that you would still see improvement as long as your surgery is done by an experienced rhinoplasty surgeon.

What stands out more to me is the heaviness of your brows related to brow ptosis. The distance between your upper eyelid lashes and the lower edge of the brows is very small, compromising the aesthetics of the eyes. Optimal improvement would be provided by a brow lift and upper eyelid ptosis repair.

To provide you with a more concise answer, I would need to know the concerns that have you have regarding your nose and what your aesthetic goals are. Nevertheless, judging by your photos, I feel that several improvements can be predictably achieved with a very nice result. Certainly, thick skin in the nasal tip reduces the degree of refinement that is created at the level of the cartilage so this must be understood.

Creating a smaller lower nose would involve trimming the tip cartilages and shaping the tip with sutures. The nose will also look smaller by reducing the the width of the base of the alae. These procedures together will allow for a more balanced look to your nose. I would do not one without the other.

Providing treatment recommendations requires a comprehensive physical examination and making the proper diagnoses. This will vary from patient to patient, and the surgical plan is tailored to the individual needs of each patient. Often, patients over the age of 60 will have excess skin or fat prolapse that needs to be addressed. Some, but not all, will also benefit from tightening or suspension of the lower lid, which may involve a canthopexy or other procedure.

You have various asymmetries involving the periorbital area but these are very mild and normal. You have a slightly greater amount of skin in the left upper eyelid that could be addressed by a conservative upper blepharoplasty. Whether this will meet your expectations is uncertain. It appears that there may be persistent fat prominence. However, being 7 months postop, you are still healing/recovering. I would recommend allowing at least a full year for recovery before judging the outcome. A revision, if you are inclined, may be appropriate if the swelling is still present.
