As an eyelid, lacrimal, and orbital surgery specialist with over 15 years of experience, he has performed more than 6,000 eyelid surgeries, including thousands of cosmetic procedures, such as upper blepharoplasty (eye lift), lower blepharoplasty, ptosis repair, and brow lifts.

In addition to cosmetic eyelid surgery, Dr. Kapadia offers nonsurgical treatments such as Botox and dermal fillers (Restylane and Juvéderm) for facial rejuvenation around the eyes, cheeks, temples, lips, chin, marionette lines, and smile lines.

Dr. Kapadia graduated from the University of Pennsylvania School of Medicine in Philadelphia. In addition to earning his medical degree, Dr. Kapadia was awarded a PhD in neuroscience for his research in visual perception. He later completed a residency in ophthalmology with Harvard Medical School at its primary ophthalmology teaching hospital, Massachusetts Eye and Ear in Boston. After finishing his specialty training, Dr. Kapadia completed a fellowship in ophthalmic plastic and reconstructive surgery at Boston Medical Center.

When he is not working at his practice or performing surgery, Dr. Kapadia teaches ophthalmic plastic surgery as an assistant professor at the prestigious Tufts University School of Medicine. He has also authored several research publications and textbook chapters in his field.

Dr. Kapadia is board certified by the American Board of Ophthalmology and a member of the American Society for Ophthalmic Plastic and Reconstructive Surgery and the American Academy of Ophthalmology.

Contrary to what most people think, fillers can often many years, especially when they are causing a problem. Dissolving the filler with hyaluronidase should help get rid of it much more quickly and is a very safe procedure in trained hands.

Hard lumps after fillers are not normal and should be dissolved. These are likely your body mounting an immune response to the fillers called a granuloma. They are best treated with hyaluronidase injections to dissolve the filler. It often takes multiple rounds of dissolving to get rid of these.
If I had an autoimmune response to Sculptra, how likely to have one to Voluma?

Sculptra and Voluma are very different fillers from an immune standpoint and unlikely to have cross reactivity. That being said, if there were a problem, Voluma can be notoriously difficult to dissolve. If patients have concerns about immune reactions, I usually recommend using Restylane Lyft in the cheeks which also works well in this area and is much easier to dissolve if there are any issues.
Pregnancy after getting fillers a while back, what are the risks?

Its not recommended to get fillers or Botox while you are pregnant or nursing, as tiny amounts of product may enter the bloodstream during injection and reach the fetus or baby. There is no known risk if you get pregnant after having these products.

Under eye wrinkles are very difficult to treat and there is no way to get rid of them completely. I would recommend resurfacing the area with a CO2 laser to help tighten the skin.

Its definitely possible to place Botox just in the procerus and not treat the forehead muscles. Usually this works fine. Once in a while, placing Botox in one muscle can cause some changes in the use of other muscles. This can always be corrected with a minor touch-up if this happens.

Your main issue is extra puffiness under your eyes. This is best treated with surgery to remove the extra fat. Fillers may help to camoflage the extra fat, if you want a milder improvement but surgery would be a much better option. You also have a mild malar bag which can sometimes worsen after filler injections.

Ptosis surgery is the most difficult type of eyelid surgery to achieve symmetry. The usual goal is to get the height of the upper eyelids within 1mm of each other. Overcorrection can be brought down with a second surgery, but there is always the risk of brining it down too much or leaving too high.

Its hard to tell how different they are from your photo as you are looking downward. The best away to assess eyelid height is with a photo from straight ahead with a camera flash.

The main issue is that there is excessive puffiness under your eyes. This is best treated with lower blepharoplasty surgery. Trying to camoflage this much puffiness with cheek fillers will only make the area look odd and unnatural.

Its hard to tell whats going on without an in person examination. It looks like you may have differences in the th bony structure on the two sides of your face, which is very common. The differences between your eyes might be improved with subbrow injections of filler.

You would need an in person examination, but I suspect the problem is more than just the brow position. Adding some filler to the the subbrow area on the hollow side should help. And yes - Botox may be used to help lower the brow, but only if you are actively pulling it up.
