Statement: Dr. Leland Deane is a board-certified plastic surgeon based in New York who’s been in practice for over 35 years. Dr. Deane is a senior partner in the New York Plastic Surgical Group, the oldest continuous plastic surgical practice in North America, with locations in Garden City, Manhattan, and Babylon. He also serves as the Associate Chief of the Division of Plastic Surgery at the North Shore/LIJ Health System at Southside Hospital and is a faculty member at Cornell University Medical College and NYU School of Medicine.

Dr. Deane specializes in surgery of the breast, body, and face, including breast augmentation, tummy tucks, facelifts, and body lifts, as well as blepharoplasty (eyelid surgery). Additionally, he offers a wide range of nonsurgical treatments, including dermal fillers and neuromodulators like Botox, chemical peels, scalp therapy, and laser skin resurfacing.

After graduating cum laude from Union College in Schenectady, New York, Dr. Leland pursued his medical degree at State University of New York Downstate Medical Center in Brooklyn. Following medical school, he completed a five-year residency in general surgery at Tufts New England Medical Center in Boston, followed by a plastic surgery residency at Plastic Surgical Specialists in Norfolk, Virginia, one of the top U.S. programs at the time. He also pursued a fellowship in hand surgery at Thomas Jefferson University Hospital in Philadelphia, Pennsylvania.

Dr. Deane is board certified by the American Board of Plastic Surgery and a former Diplomate of the American Board of Surgery. He is a member of the American Society of Plastic Surgeons (ASPS) and the American Society of Aesthetic Surgery, as well as a Fellow of the American College of Surgeons (FACS). In addition to a medical degree, Dr. Deane also completed an MBA at New York University’s Stern School of Business.

When Dr. Deane is not helping his patients reach their aesthetic goals, he is educating future generations of plastic surgeons through speaking and authoring numerous papers in the plastic surgery literature. He has also helped organize and lead missions to Colombia, South America, where he surgically treats congenital malformations in children who may otherwise not have access to such procedures.

Without preop photos it is a little difficult to answer your questions, so I'm making assumptions about what you were like before. So, these are my opinions only. These implants (and the ones you are thinking about putting in place) are way too large. Breast augmentation is a progressive stretch of the tissues and 'dropping' (really a misleading term) will mean that you stretch perhaps in the wrong way with poor placement, position and a strange looking shape. Yes, I feel that you do indeed have a type of symmastia, giving a sort of 'web' between the breasts. Breast augmentation is still possible and may look great, but the dissection has to be very carefully done ( and not 'overdone'). Suggest you choose a surgeon with a lot of experience.

I would suggest augmentation alone. Your breast configuration and nipple position do not warrant, in my opinion, a lifting procedure. You should do well with augmentation. "Safe" is open to wide interpretation, and in general terms will be defined differently depending on the surgeon you go to, the state and federal regulating bodies governing the venue and doctor and staff, and your tolerance for risk. It's all up to you. Once you leave this country for surgery, you also leave all the medical and legal protections that you might take for granted. You will have to decide if the cost savings are worth the risk. And....should you have a problem from your surgery...

Realize that there is no one 'right' answer and also you may have signed a permission form for surgery that stated somewhere that secondary surgeries were the financial responsibility of the patient. Also, you had a difficult preop problem(s) and I would have warned that secondary surgeries are likely.

I agree with the comment that too often breast width is used to determine implant size. This method is a holdover from the early days of saline implants. You have to be careful that you get a nice shape and not create distortions for the sake of 'cleavage'. You definitely need a lower eyelid incision, but this is ok because the incision itself should heal well. The critical maneuvers, in my opinion, are to provide support to your lower eyelid and orbicularis muscle which is sagging. Some fat is either removed or repositioned as well. You are definitely at higher risk. The real question is how the surgeon will lessen that risk and make it acceptable. The key thing here is to get a beautiful breast shape and I certainly think that this is possible using an experienced surgeon. My best advice based on your photo is not to have an implant at all, because this would not add much in terms of overall result and could actually harm your eventual outcome.
