You do have a small amount of residual ptosis on the left.  Often a surgery can be done on the underside of the eyelid to lift the lid, called a muller's muscle resection.  In the office a drop is placed in the eye to evaluate how well the muscle works and to help us decide how much to excise.  This is usually the best way to lift 1-2mm.

Radiesse is not a permanent filler.  The filler stimulates new collagen, a bio-stimulatory filler, but the product itself dissolves with time.  You can do HIFU over the area.  From the picture it is hard to tell what you are unhappy with.  HIFU usually best to help with jowls, submental fullness and lax skin.

Ulthera should not be used inside the orbital rim which is where the fat is located.  RF may help to tighten the skin and might make them less noticeable.  Filler put in below the fat pads can often make the area appear smoother.  Or surgery is always a good option for prominent fat pads.

Most of what you have going on here is a heavy lateral brow.  If you take off some skin from the lids you may look about the same.  If you elevate the lateral brow this would open up the outer part of the eye.  You could try some Botox first, and if you like the way it looks with the brow higher, you can consider a surgical temporal brow lift.

An upper eyelid surgery is one that can usually be done easily with local sedation.  You can also have some pills for anxiety if needed.  Your eyes are closed so you won't see what is actually being done.  The only discomfort is the injection of local anesthesia. For patients who are too anxious to do it this way, you can have IV sedation.  It is your choice.      
Can you get Facetite or Accutite after facial fat transfer?

I would not suggest these after a fat transfer.  Heat is delivered under the skin with the facetite probe, and this will melt some fat and stimulate new collagen growth.  So you may lose the fat that you just put in there.  Might want to try Microneedle with radiofrequency as there should be less fat loss with this, and it can also stimulate new collagen. There are many machines out there to do this.

In this photo it appears that you have a significant ptosis of the upper lid.  You should see an oculoplastic surgeon who can evaluate the eyelid muscles to determine what you have.

In these photos you have ptosis of the upper lids.  Brow height seems to be okay.  You should see an oculoplastic surgeon who can evaluate the eyelid muscles to determine what would be the best surgery to elevate the eyelids.  May want to remove some excess skin as well.

Tough to tell much in this picture since you are looking down, but may have some ptosis again.  What you comment on is the lack of a definitive crease - you have a couple of them.  If an incision is made in the upper lid at the crease, sutures can be used to both fix any ptosis and give you a defined crease.  See an oculoplastic surgeon as they are specialists in this sort of surgery, especially in secondary surgeries which are more difficult.   
My eyes are not the same size. Is there any treatment for this?

You have a mild ptosis of the right upper lid.  This causes the right brow to elevate more to try and lift the lid.  A ptosis repair surgery will give you more symmetry. 
Fat graft or fillers for under eye hallows? Which is more effective and long lasting?

You have a deep tear trough which makes the area appear dark.  I would do HA filler first as it should work well, will last at least 1.5-2 years, and if for some reason you don't like it you can have it dissolved.  Also no surgical procedure needed as you would for fat harvesting.  There can be more complications seen with fat as well.

If you truly have no fat to transfer, you can add some fillers after the facelift inflammation has subsided.  If you do the HA fillers at the same time as the facelift, they will probably dissolve pretty quickly due to the inflammation of the surgery.  So wait about 6 months. 
Ptosis? Eyelid or brow lift?

You have an eyebrow asymmetry.  This is often associated with ptosis of one of the eyelids, but that does not seem to be the case for you as you do not appear to have ptosis in these photos.  Botox can be used to elevate one brow and lower the other,  but you state this has not helped.  Your only other option is a surgical browlift for the lower side.

You have a very full heavy brow which most people consider attractive.  If you have some Botox placed into the temporal brow,  you should get an elevation of the lateral brow which will make it appear less hooded. 
Are my eyes extremely asymmetrical enough to have surgery?

You have an orbital asymmetry - the bony orbit is lower on one side.  This would be a very difficult thing to fix, and as you said, most people don't notice this on you.  So I would recommend you do nothing.  If it bothers you, wearing glasses may hide the asymmetry.
