Thanks for reaching out and sending your photo. It’s always difficult to fully evaluate an eyelid without being able to touch the lids and see it in 3 dimensions. That being said, I agree that a pinch skin excision with give you the biggest improvement. I would also probably add a filler also to help with the dark circle. Laser won’t be enough. I say that a pinch (and possible lower lid tightening with it) will improve the lines maybe 30%. A laser may improve the lines like 5%.So pinch skin…

Yes, filler can be placed here. It is extremely challenging. Very few doctors fill this area. You would want to see an oculoplastic surgeon for filler to this area. Your upper lid skin is slightly thinner than average which makes the filling in this area even more challenging. Given those 2 challenges, I would place some filler (Refyne), then let the swelling subside, then place the finishing touch on it with the remaining filler a few weeks later. Best of luck to you.

A skin pinch to remove excess skin on the lower eyelids will not make the eyes look smaller. A canthoplasty, which is on rare occasion done with a skin pinch, is what can make the eye look smaller or can make the eyelids look slanted upwards or slanted downwards. Your upper lids and brows could be made similar to the 2nd picture. Would need ptosis repair (a surgery) of the upper lids, botox, and fillers. However, I think that your eyelids look very pretty. A ptosis repair alone would make you look very awake and alert. After that, you could consider if you need botox or fillers.

You have left upper lid ptosis. A left upper lid ptosis repair will improve symmetry. You also have 2 creases on your left upper lid, so it will be important to pick the correct crease in order to attain symmetry with your right upper lid. You do not have hypertropia. See an oculoplastic expert.

Yes, you have a muscle that pops up when you make facial expressions. The treatment for this is botox injections. After a few botox injections over time, the muscle with slim down and pop out less. Surgery is not used for this. See an oculoplastic expert.

You have ptosis on the right upper lid with extra creases and folds (your 3rd photo makes it look like it is your left eye, so I presume that it is mirrored). Yes you can have a 2nd surgery to improve the ptosis and improve the creases and folds. Upneeq will raise your eyelid for around 6 hours and then it will drop again; it is not a permanent solution; it is good for mild ptosis or temporary ptosis or people who do not want to have surgery. You would probably still have the extra creases…

I see the asymmetry that you are talking about. It can be due to ptosis of the left upper lid, or it can be from a loss of fat from the left upper lid which is forcing the crease higher and/or making it fold differently. You need an in person exam with an oculoplastic surgeon.

Hi Debsmills, you do have a mild to moderate ptosis and a lack of orbital fat. A ptosis repair will have some improvement, although probably not enough to completely fill the hollow. A phenylephrine test will show you how much improvement you will probably get. Then do the ptosis repair. If there is any residual hollowness that you do not like, then add filler, but you would go to a very specialized oculoplastic surgeon for this because not everyone does these injections. Not to worry. You will not have bad effects from lying down after botox. Not lying down for 4 hours after botox is what I consider a old wives tale. I do not restrict my patients from lying down after botox. Your lightheaded-ness is called a vasovagal reaction, which is very common, so no need to feel embarrased. On the photos, it’s hard to tell for sure because you are not looking directly at the camera lens. Also with photos, so many cameras will mirror the photo so it’s unclear which is your right eye or left eye. Presuming your camera did not mirror the photo, the left upper lid has more of a “double eyelid” while the right upper lid has more of a single eyelid.
