Hello and thank you for your questions and photos. You have found out the hard way that threadlifting is very temporary and less than adequate to reduce submental skin laxity in the aging face. The procedures that would help you obtain your goal would include a mini facelift with submental platysmoplasty. My favorite is the “S” lift pioneered in Europe for use even in the smoking population. It includes a short incision in front of the ear and stops just behind the earlobe. ML

Hello and thank you for your question. In over 35 years of practice, I have only removed 3 submaxillary glands due to ptosis of the gland and often times a reactive lymph node that is adjacent to it. Of the 3 glands, two were removed from the same patient. Nothing takes the place of an in person consultation. With a personal evaluation of your condition, the surgeon has a better opportunity to make a plan that is predictable. Malpositions in general are repairable and with resurfacing and hiding the scars in natural occurring grooves and folds, one can quite often camouflage the scars. Make sure your doctor has experience with trauma patients.

Yes there is a procedure; a mid facelift. At your age, I would advise against it and try to correct your problem with a longer lasting filler. I have found Bellafill to help correct tear trough deformities and help revolumize the lower lid region. This filler lasts over 5 years and will help with the pigmentation issue. From the least invasive to the most invasive treatments, I would review the results obtainable from a CO2 Fractional Laser versus the Ulthera.
