The best option for addressing the fullness in your neck would be a deep plane necklift, together with some liposuction. You are unlikely to be happy if just liposuction is performed without addressing the deep structures.

This could be combined either with an extended deep plane necklift or a deep plane facelift to address the resulting skin excess.

In men, my preference is to use Terino Square Chin Style I implants. I find the width in the front gives a more masculine look. I will not put in the extra large size implants however. I feel that very large implants put more pressure on the bone, and can lead to bone resorption and loss over time. In patient’s who desire a large sized chin implant, I prefer to use MedPor chin implants instead.

Based on these photographs, it appears that you have had a fairly good result following surgery. The severity of the gynecomastia prior to surgery also determines how extensive of a correction was required, and what needed to be done. Close up photographs would be required to better see the issue you are describing, and to see if anything such as a scar revision or a different revision surgery could be performed to further improve it.

Bra Sizing is Very Inconsistent

Unfortunately, bra sizing is very inconsistent. Some women can go to three different stores, and leave with three different sized bras. As a result, many women may be wearing the wrong bra. It would be reasonable to remove the implants and then perform a BBL, but this should not be done in one surgery. I would suggest removing the implants first and allowing things several months to settle, and then doing the BBL at a second stage. In your case there will be more scar tissue present, and precautions should be taken when performing a BBL. The safest way to perform a BBL would be under ultrasound guidance to ensure that no fat is placed into the muscle.

Without knowing when your surgery was performed, I would suggest seeing your original surgeon back to see if anything can be done in the meantime. If a seroma is present, it can be aspirated or a small drain can be placed to help it to flatten out.
