It is possible to shorten the chin with a reduction genioplasty. The amount of projection can be kept the same. The actual height should be determined from a CT scan or cephalometric x-ray.

In general, silicone implants are easy to remove. This can usually be done in the office with local anesthesia. The potential problem is the possibility that the soft tissue pocket may not return to normal if the implant has been there for a long time. In younger patients, this is generally a less frequent problem.

You have a very recessive chin based on the lateral ceph. I would recommend an advancement genioplasty of 9-1- mm. That is the extent of advancement that can be safely performed while maintaining bone contact. It does not appear that you will need a bone graft and the vertical height looks within the normal range.

It is possible that the implant has migrated due to lack of screw fixation so I would get a CT scan to evaluate the implant position. Once an implant has been in for a few months it will become encapsulated and therefore should not move unless it has migrated off of the bone. It can easily be removed and replaced or even repositioned.

Yes, the sliding genioplasty would be best in your case. The chin can be advanced 9-10 mm. In some cases, I place a chin implant later if more projection is needed (cases that need 15 mm). You do not need to undergo corrective jaw surgery if your bite is fine. The sliding genioplasty is performed in the office with sedation anesthesia and takes about 45 minutes. There is no change in your bite or chewing. Most patients return to work in 3-5 days.

The sliding genioplasty would lengthen your chin and diminish the depth of the labiomental fold but it will not impact your facial balance in a negative way. In the one photo, it appears that you are deficient vertically and horizontally. You have a deep mentolabial fold which tells me that you need vertical lengthening. A sliding genioplasty can provide both lengthening and advancement with one operation. Vertical lengthening of the chin cannot be adequately accomplished with a chin implant. It tends to deepen an already deep fold. The procedure should improve the balance of your lower face.
