Dr. Rand earned his undergraduate degree from Stanford University and his medical degree from the University of Michigan in Ann Arbor. In his postgraduate studies, he completed two residencies: one in General Surgery at Tufts Medical Center in Boston, and a second in Plastic Surgery at Emory University in Atlanta. He also completed a fellowship in Craniofacial Surgery at Miami University in Ohio.

Today, Dr. Rand is double board certified by both the American Board of Surgery and the American Board of Plastic Surgery. He is also a member of the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS), the American Society for Aesthetic Plastic Surgery (ASAPS), and the American Society of Plastic Surgeons (ASPS), as well as a fellow of the American College of Surgeons.

I agree with Dr Baxter’s opinion. You could also go to a higher profile implant which is built to give more upper pole fullness. You absolutely do not need a breast lift because your nipples are not low at all.

Revision for implants should I downsize or have them removed with a lift?

It is not uncommon to have some discomforts/pain long after implants are placed. You should get a mammogram and ultrasound to be sure your breasts are healthy and that the implants are intact. Usually the pain just goes away over time. The ONLY way you would need a lift after implant removal (can’t tell from the photo) is if your nipples are down to or below the crease under the breast.

You could do better with your weight first and that will help reduce any fat inside the abdomen that might be contributing to the fullness. This is fat the surgeon cannot do anything about, only weight loss fixes that. Second, it looks like a full redo might be needed as there is still too much skin. Your result looks excellent, sorry you are not happy. Please don’t let this wear on you at all. Most would be very pleased with your result. If you wanted to go bigger, you will need to absorb more costs because the surgeon did nothing wrong and you need to know that if some rare complication like an infection occurred, you would need to remove the effected implant and start all over a few months later with the chance that it will never be right because of scar tissue.

The biggest issue is that until 22 you need to be saline. The silicone gels feel much more natural and are worth the wait for most. Insurance (general medical) doesn’t cover implant ruptures. That insurance comes from the implant companies and doesn’t matter your age.

It is best for patients not to fall for advertising campaigns from implant manufacturers as you are not expert at this and have no experience or follow up history. You have an exceedingly subtle double bubble where the old IM crease is indenting the breast above the new crease. This probably came from lowering the crease to fit in the large implants. No fix without surgery but yours is so small you should let it alone for a year to see if it doesn’t stretch out to satisfsaction. Discuss with your surgeon.

It is a mini TT that does nothing above the umbi. But a properly done full TT undermines the skin all the way up to the bottom of the breast bone (xyphoid) and this releases the upper ab tissue to be pulled down. The effect is stronger in the lower abdomen but the upper does benefit too.

Mostly, saline above the muscle is not a good choice for exactly your issue. Unless you have enough thickness of tissue you WILL see and feel wrinkling. Your limited photo suggest that you are thin and have large implants that are too wide for your body and that may have further thinned your tissue. A submuscluar conversion with a high fill percentage gel would make it much better. But seek in person consultations for sure.
