Dr. Richard Zinn, FRACS 3002
Dr. Zinn studied medicine at the University of Cape Town before completing a research-focused master’s degree in lipodystrophy, the study of abnormal fat distribution. He then went on to complete multiple residencies and fellowships, including one in microsurgery, before becoming a Fellow of the Royal Australasian College of Surgeons in 2016. Dr. Zinn frequently travels internationally to attend plastic surgery conferences and training workshops to ensure he offers the most advanced techniques. He passes along that knowledge to future generations of surgeons in Victoria at the Royal Melbourne Hospital and Peter MacCallum Cancer Centre.

I would recommend doing minimal internal cleaning and removing of crusts. They will come away with time. I would warn against excessive internal manipulation where you cannot see as you may interfere with sutures, cartilage graft and the septum. After the surgery, sensation may be reduced so if it does not hurt, it does not mean it is ok.
My advice is to follow your surgeons recommendations closely. Use saline nasal spray and topical…

Make sure you do not expose your incision site to any laser or chemical peel as this will likely interfere with your scar healing in this area.
Aslo, confirm that your surgeon is happy with this as he should really be informed of this if you decide to go ahead.

A tissue expander would be employed to stretch the skin, and in itself may bring about more stretch marks. In any event, this would need a number of operations, a high complication…

I would suggest reviewing with your surgeon who would be eager to manage this problem before it gets worse. It is also something your GP may be happy to dress if he has an interest in wound care.

Some seromas need a single drainage to settle. others need recurrent drainage before it settles . My advice is that while you suffer with a stroke, see your surgeon weekly. You may have a combination of swelling and seroma and so once the seroma is removed, residual swelling…

You ask an interesting question – this is the holy grail of plastic surgery – how to defeat gravity. Surgeons have tried many options – anchoring sutures in the clavicle, internal mesh bra sutured to clavicle and ribs, breast held up by muscle strips.
Unfortunately, if you have thin skin and soft tissue, you are likely to resag. It sounds like this is happening. You may need another lift in a few years and I would not recommend any of the options…
