After looking at your photos a lower lid bleph is a good option and perhaps a little fat grafting into the tear trough after releasing it.

Do I need a full face lift or just neck? I had a mid lift 10 years go. I am 61.

After looking at your photos I would recommend doing a full face/necklift.Also I would do some judicious fat grafting to your face to fill in some of the hollwed out areas.I use fat almost 70% of the time when I do facelifts.

After looking at your photos you look really good.I see some separation of your platysma muscles in your neck creating bands so you could just do a necklift and leave the facelift for another time or do them together.

After looking at your photos a breast lift would help and a small implant could be done at the same time.Will they sag again down the road?For sure.It is not permanent but will be much better.

After looking at your photos you have an early separation of your platysma muscle which is causing the neck band especially on the right side.I think you are a bit young for a neck lift but that would be the best solution for correcting this.Ultherapy is a waste of time and money.

Can someone explain in detail how a platysma myotomy is performed? Would I be a good candidate?

Without photos it is hard to say what needs to be done.You have tried all the non-surgical things to do.At this point I don’t feel a myotomy which is a cutting of the platysma muscle will help you in this regard.I would follow up with your surgeon and discuss this with him.
