you appear to have small fat festoon with volume loss and a negative vector. I urge you to be cautious with lower eyelid skin removal. Pinching the skin is not enough of a cause to remove it. Perhaps an evaluation for a fractionated ablative laser could help with volumization. Please seek a qualified oculoplastics specialist for this issue.

Be patient, it’s a bit hard to dissolve voluma and don’t be surprised if it takes a few attempts.   When completed, I would give your face a little time before re injecting.   At that point it is very acceptable to switch to Restylane-based products.

 From the photo it appears as though you have left upper eyelid ptosis with a small amount of left brow ptosis. I would have to look at all photos to evaluate whether it is truly left upper eyelid ptosis or right upper eyelid retraction. Contrary To what was written as a response. And upper eyelid blepharoplasty will not correct upper eyelid ptosis these are two separate issues. You should seek the evaluation from a qualified oculoplastic surgeon.

Thank you for your question, I get this asked pretty often. Losing weight will not affect your eyelid fat bags. This was fat that was previously behind your eye that Slowly came forward as the septum weakened.   You can have your lower eyelid blepharoplasty comfortably regardless of weightloss.

This is a great question.  I would look for someone specializing in eyelid surgery.  Someone who does a lot of this kind of surgery and someone who can understand the nuances of different skin types and anatomy.  Everyone’s skin, facial anatomy and aesthetic goals are individualistic which is what makes the specialty so interesting and exciting.  A lot has changed over the last decade to really provide specialized care.

I hear this question often.  When smiling muscles come into play.  I would have to see a better picture, straight on (not to side) and one not smiling.  Generally when there is a difference  between the two eyes while smiling it is from a hyper active or less commonly an under active orbicularis  muscle (muscle that closes eyes) on one side as compared to the other.  There are of course circumstances where one would have eyelid ptosis or dropping but that is usually seen when not smiling.

This is a situation where an in person evaluation is crucial.  It appears you have a negative vector (eye sticks out far past orbital rim) this making your lower eyelid support compromised.  It is unusual for a canthopexy to be enough.  The whole lower eyelid needs to moved upward, only than can the corner (canthus) be a sharper angle.  I am least worried about the scar And most about the eyelid position itself in these cases.  I can not tell enough from photos, these cases need to be seen ...

I agree you would benefit  from feeling the periocular region. Second of thing would be a good option as well as more Restylane filler. While I love using Sculptra Elsewhere in the face, I do not believe this product belongs  around the eyes. You would benefit from an in person evaluation. I believe  either of those two options I just mentioned to fill the region should work well.
